Job Description:

An exciting opportunity for RN Case Manager has become available at an award-winning acute care hospital located on the outskirts of Metropolitan Texas.

This 159-bed facility delivers high-quality care with the most up-to-date and innovative medical technology. This well-known hospital is dedicated to providing safe and effective patient-centered and individualized care. Some of the specialties offered include surgical services OR, women’s services OB, neurology, orthopedics, emergency services ER, and cardiology.

The RN Case Manager will coordinate the plan of care for patients from admission to discharge. The RN Case Manager will act as a liaison to ensure optimal care is provided in a timely manner. The RN Case Manager will work collaboratively with leadership and will demonstrate knowledge of standard concepts and procedures. The RN Case Manager is supported by a team of Physicians, Advanced Practice Providers, Registered Nurses, and staff.

Requirements include:

• Minimum Associate Degree


• Experience working case management


• Ability to work collaboratively and communicate effectively
