KEY JOB RESPONSIBILITIES: Provides constant and ongoing care for elderly and terminally ill patients by evaluating patient needs, creating care plans, and providing end-of-life support to patients and their families. Works in a collaborative, team-driven environment that requires frequent communication with other Care Team members.

Additionally, the Registered Nurse will:

• Registered Nurse/ RN regularly assesses and reassesses the nursing needs of the Hospice patient.
• Registered Nurse/ RN provides dietary counseling.
• Registered Nurse/ RN provides Hospice nursing services, treatments, and preventive procedures.
• Registered Nurse/ RN initiates nursing procedures appropriate for the patients Hospice care and safety.
• Registered Nurse/ RN observes signs and symptoms and report to the physician and IDG members any unexpected changes in the patients physical or emotional condition.
• Registered Nurse/ RN teaches supervises and counsels the Hospice patient and family members about providing care for the patient.
• Registered Nurse/ RN supervises and trains other nursing service personnel.
• Registered Nurse/ RN develops and re-evaluates the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care.
• Registered Nurse/ RN performs specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctors orders.
• Registered Nurse/ RN attends team conferences.
• Registered Nurse/ RN follows the policies and procedures of The Care Team. Observes confidentiality and safeguards all patient-related information in compliance with HIPAA regulations.
• Registered Nurse/ RN always communicates to the supervisor if unable to meet a patients need or perform a procedure.
• Registered Nurse/ RN participates in on-call system and is responsible for providing on-call coverage when unavailable for assigned duties.
• Registered Nurse/ RN maintain skills and knowledge.
• Registered Nurse/ RN coordinates the implementation of the plan of care for patients residing in SNF, NF, ICF or MR. HOS RN FACILITY POC
• Registered Nurse/ RN organizes work schedule and utilizes time management to be able to attend all required meetings.
• Registered Nurse/ RN complies with agency infection control policies and protocols.
• Registered Nurse/ RN assist with orientation, teaching and training as requested.
• The clinical record shall contain notes for each service provided. A clinical note will be completed for each visit made to a patients home (including private residence, assisted living facility, group homes or skilled nursing facility) within 48 hours of the visit.
• Other duties as assigned by Regional Branch Director.

Professional Experience/Qualifications:

• Maintains the agency's mission, philosophy and core values.
• Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety and emergency management.
• Always maintains patient confidentiality, including all HIPAA regulations

Regulatory Requirements

• Licensed or registered in the state in which he/she currently practices
• Must pass a criminal background check & MVR check.
• Completed health statement acknowledging ability to perform the duties of the position.
• Valid state drivers license. Proof of current automobile insurance.
• TB testing per agency policy; (1 or 2 step TB skin test within 12 months of hire & annual TB symptom screening thereafter).
