Caregivers of New Jersey is an affiliate of The Family Resource Network. We are a statewide agency devoted to helping individuals with intellectual/ developmental disabilities and their families with their case management needs.

JOB DESCRIPTION

Coordinator Assistant

Reports to: Administrative Assistant

Job Summary: Works in partnership with Support Coordinator team to manage a caseload. Assists qualified individuals with I/DD, and their families/guardians, identify and obtain the needed services and supports to support their lives within the home and in the community.

Essential Job Functions:


  • Ensures that all work is carried out in a manner that is both respectful and useful to individuals, and their families/guardians, with intellectual and developmental disabilities.

  • Works closely supporting Support Coordinators

    • Coordinating Assistant’s will be required to attend the 2 day Boggs Support Coordination certification orientation training, in order to assist the Support Coordinator with their monthly deliverables.   

  • Answers in-coming phone calls daily, and by remote by Ancero.

  • Provides the Support Coordinator with appropriate referrals

    • For initial home visit based on the initial phone call information collection (if needed)

    • Generic Supports

    • DDD Qualified Providers

  • Uploads required information into the I-Record and Foothold Notes

  • Process Revisions to the POC as instructed by the Support Coordinator

    • Cut & Paste for ISP/PCPTs

    • Updates data in I-record and/or Foothold

    • Submits to SCS for approval

  • Mails all documents, to providers and customers, within 3 working days, upon approval via US Mail and via I-record to billing departments.

  • Will utilize the tools provided for password protecting documents, compressing, uploading when required.

  • Fax and Scan documents when required.

  • Reviews all documents, records notes and updates/uploads all records in required format (outlook, foothold, etc.) as needed.

 


  • May connect the person/guardian/family to service providers (contracted and/or generic). Includes setting up pre-visits to programs and, meet and greets which may include the Support Coordinator.

  • Maintains continuing contact with individuals and families as individual plans of care are put in place

  • Assists individuals and their families in coordinating, selecting, and overseeing their own supports.

  • Informs providers when services have been approved or are ending.

  • Completes all assigned monthly phone calls for Monitoring Tools by the 15th working day of each month. 

  • Schedules quarterly visits and follows up with Support Coordinator for all monitoring tools accordingly.

  • Completes required trainings, required annual professional development, and attends meetings as assigned.

  • Able to travel to appointments, anywhere in NJ, when needed, when a Support Coordinator is not available.

  • Attends Marketing a minimum of 4 marketing events each fiscal year.

  • May perform other duties as assigned by the CEO, SVP of Programs and Services, Director, SC Supervisor, and Administrative Assistant/Supervisor as need arises.

QUALIFICATIONS/ BACKGROUND:


  • Bachelor’s Degree

  • At least one year of experience working with adult individuals (18+) with intellectual or developmental disabilities.

  • Proficient in MS Office Outlook, Word, Excel, Power Point;

  • Able to solve problems independently, effectively, and creatively.

  • Excellent Organization and Communication skills;

  • Excellent time management skills to be able to juggle multiple plans for multiple families simultaneously.

  • Must have a flexible schedule to meet the needs of our customers;

  • Bilingual strongly preferred.
