Job Description:

Holistic Well-Being Navigator

The Holistic Well-Being Navigator will build trust and rapport with employees and leaders at their dedicated worksite to help facilitate culture change in all areas of well-being. This client-facing role will be onsite full-time with the ability to travel to any of customer's properties at request. The navigator will serve as a wellness benefit concierge, providing referrals and education that support both emotional and physical well-being. The navigator is committed to addressing the employee's barriers to care including social determinants of health by helping the employees navigate and schedule all healthcare appointments or connect to community resources to meet their needs. The navigator will exhibit competency in providing culturally sensitive care to a diverse group of employees. In partnership with the customer's wellness team, the navigator will implement and execute effective wellness solutions to close gaps in care and reduce total plan spend.

Responsibilities

Customer Engagement


  • Proactive and responsive to employee needs in a friendly and professional manner

  • Liaison between employees and customer's wellness education programs, campaigns or well-being initiatives at the worksite

  • Perform needs assessment surveys to employees to identify and address healthcare barriers

  • Provide referrals and social support to appropriate well-being resources, programs, and health care services

  • Follow-up with employees on referrals to ensure individual needs have been met

  • Engaging thoughtfully to help individuals that don't know they need care or exactly what care they need

  • Complete required record keeping for tracking results and provide feedback on program goals and objectives in a timely manner

Wellness initiative campaigns communication


  • End to end coordination and communication of all well-being events

  • Promote events via customer approved communication channels i.e. intranet, QR codes, etc.

  • Distribution of collateral to local leaders, resource centers, and other areas

  • Leveraging data and emerging trends open to suggesting and implementing improvement on needed events to engage the evolving needs of the population

Health Awareness Presentations


  • Deliver single or multi-session workshops on a group basis as requested and based on emerging needs of the population

  • Develop or pull from defined list of wellness topics; may be tailored to population, season, etc.

  • Education for various health related topics (nutrition/healthy eating, physical activity, etc.)

  • Facilitate health related group activities among population over a defined time period (walking groups, weight loss support groups, etc.)

Qualifications


  • Spanish or Haitian Creole Bilingual to support customer's population

  • Bachelor's degree in health education, health promotion, or health related field

  • Minimum of 2 or more years of experience in workplace health and wellness programming

  • Understanding and sensitivity to multi-cultural communities

  • Excellent communication and interpersonal skills

  • Strong organizational and time management skills

  • Ability to document patient interactions in multiple electronic systems

  • Work independently with critical thinking skills

  • Strong team player and proactive problem solver capable of accomplishing program goals

  • Ability to maintain member confidentiality

  • High degree of patience and commitment to team culture

  • Willing to be flexible in work schedule (occasional evening and/or weekend hours)

  • This role is based on-site in Anaheim CA.
