Job Description:

JOB SUMMARY:
This position will ensure that the health and safety of all residents are met. The LPN will assist the Director of Nursing with planning, coordinating, and managing the home care service and act within the Comprehensive Home Care regulations. The LPN is responsible for the overall direction, coordination, and monitoring of home care service delivery under the direction of the Director of Nursing. This position will assist and lead projects outside of the normal scope as assigned by the Operations Manager and the Director of Nursing. 12-hour shifts.
QUALIFICATIONS:

Must be licensed with the State of Minnesota as a LPN, and must maintain current and unencumbered licensure.
Must be able to lift 20lbs, Stand, Sit, or Walk for long periods.
Must have reliable transportation to and from work.
Support the company values and maintain a values score of 4+.
Must be able to pass a criminal and/or abuse background check, and drug screening.

Must also meet at least 2 of the following criteria:

5+ years' experience working as an LPN in a direct care heath setting and/or
Previous experience supervising/team lead and/or
Previous experience working with tube feedings, ventilators, tracheostomy and/or
Previous experience working with mental health and/or
Previous experience leading team trainings and training unlicensed staff.
