Beacon Hill Primary Care seeks to hire a full time Nurse Practitioner ("NP"). The NP will be responsible for managing a panel of patients as well as assisting with co-managing the patients of other physicians in the practice.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
The NP will deliver Primary Care and Urgent Care to the practice's patients. Specifically, the NP will Collaborate with MD partners, primary nurses, medical assistants and support staff to deliver expert team-based care.
Manage her own panel of patients.
Co-manage patients within the practice, either in the form of providing urgent care, follow up care, or an annual visit.
Perform and record the patient history and physical exam; form an assessment and plan, which may include ordering appropriate testing and imaging, consulting with specialists, or discussing findings with a practice physician when appropriate.
Follow and manage patients (in collaboration with MD), enhancing continuity of care.
Act upon labs, imaging, and other studies promptly.
Respond to patient questions and messages within requisite time frame.
Perform medically delegated aspects of patient care as agreed upon by nursing and medical administrators according to practice guidelines.
Maintain and practice in accordance with practitioner guidelines per state and hospital regulations.
Maintain and update clinical knowledge and skills based on current health care practice/research.
Maintain the standards and educational requirements specific to the position and licensure.
Complete patient notes within requisite time frame.
Evaluate the outcomes of patient care, using patient satisfaction information, MWI, ED & readmission rates and other data as made available by Primary Care Administration, MGPO and other appropriate sources within the institution Participate in and reinforce all practice-based process improvement efforts.
Serve as a resource to nurses, medical assistants, and staff for clinical questions/problems and participate in care when indicated.
Promote mutual cooperation, support collegial relationships among all staff.
Promote an environment that is sensitive to the culturally diverse needs of patients and employees.
Masters degree in nursing required. This position requires 5-7 years clinical experience as a Nurse Practitioner, preferably with expertise in primary care. A NP license is required.
SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:
Extensive clinical in primary care, urgent care, and chronic disease management.
Experience managing patients alone and co-managing patients with physicians.=Outstanding organizational skills and independent problem-solving and decision -making abilitiesDemonstrated ability to effectively interact with physicians and coworkers.
Strong written and verbal communication skills
Experience with common PC computer software including Microsoft Word, PowerPoint, Excel, Access and Outlook; experience using clinical data capture systems such as EPIC.
