Overview

The Certified Nursing Assistant provides Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) as assigned in an individual's care plan. Care is generally provided in the home, however, errands, and escort services may also be assigned as designated in an individual's care plan. The CNA may also provide skilled tasks that require special training and competencies that a Homemaker cannot provide.
Responsibilities

Comply with all policies and procedures set out by the Hope Nursing Home Careand the CBHomeCare Employee Manual that is provided at orientation or anyadditional materials provided during the term of employment. Position is responsible for providing services as assigned by the RN FieldSupervisor in accordance with the individual's approved service plan. The Certified Nursing Assistant is required to report any deviations in anindividual's functional or medical condition to the appropriate agency personnel. Services must be provided during the assigned schedule for each individual. Deviations from the assignment including, but not limited to the following must bereported to the agency.o Deviations from the assigned scheduleo Individual declining specific tasks or requesting unassigned taskso Individual expecting to be unavailable for a shift or multiple shifts.o Hospitalizations, Rehab, or Facility placement.o Incidents, accidents or other events that are not aligned with theindividual's plan of care.

The Certified Nursing Assistant must be able to demonstrate competence withassigned ADLs and IADLs or special tasks and perform these tasks in line withthe plan of care.o Tasks may include, but are not limited to:

Bathing (assist, bed bath, tub bath, shower, sitz)

Dressing / Undressing Mouth care Skin care (non-prescription) Hair care Nail care (does not include nail care for clients with Diabetes) Mobility Turn and position Transfer Use of lift Assist with ambulation Range of motion Toileting Bedpan/urinal Bowel/bladder training program Collect specimens Perineal / catheter care Apply/Remove Condom Catheters Ostomy Care Eating Meal preparation according to client's dietary needs orrestrictions, including forcing fluids and restrictions. Assist, monitor or feed the client, according to level of care. Oral/Nasogastric feedings (after placement is verified by RN) Gastrostomy Feedingso IADLs may include but are not limited to:

Cooking Cleaning Shopping Transportation Escorto Special Tasks

Vital signso Temperatureo Pulseo Respirationso Blood Pressure Height and Weight Application of heat/cold Preventative intervention to prevent decubitus ulcers Apply TED hose, as instructed Deep breathing/cough techniques

Remove EKG leads

o Companion or Sitter services, as assigned in the care plano In addition to the above tasks, the CNA may be required to provide othertasks that are required by client's needs and level of care (proof ofcompetence required prior to services rendered.

All tasks must be performed with Infection Control in mind. These precautionsmy require mask, gloves, and gown. Handwashing and other appropriate precautions should be done before and aftertasks. Assure that the home is save from obstacles that may cause falls If additional services are requested, the CNA must notify the RN Field Supervisorfor further instruction. The Certified Nursing Assistant may not negotiate additional tasks, servicelocation, or schedule changes with the individual. The office must be notified forany alterations, failure to do so may result in termination. Be present for Supervisory Visits to be performed by RN Field Supervisor asrequired. Accurately report time and attendance to branch office. Time and attendance must be logged in via timesheets, or EVV, as required bypayer or Hope Nursing Home Care. Required documenting and reporting as assigned by RN Field Supervisor Immediately report concerns of Abuse, Neglect or Exploitation to the office Cell phones may not be used during assignment, unless approved for EVVservices, as instructed Report planned absences to the office as soon as possible so the scheduledoutage may be reassigned to another CNA. Be an advocate for individuals needing other community services or a high levelof care. Dress appropriately and professionally. Wear nametag at all times during assignment For infection control purposes, Personal Protective Equipment should be usedappropriately during assignment. Comply with rules and requirements with specific programs, as assigned Other duties, as assigned.
Qualifications

Proof of current CNA certification. Eighteen (18) years of age or older. An individual's BCI check complying with RI state law. Experience in Home and Community Based Services, preferred. Current Tb Skin Test

Proof of vaccination Punctuality Desire to help those at home Professional demeanor and appearance. Have reliable transportation and proof of current automobile insurance. Can demonstrate competence providing assigned tasks
