As part of our dynamic nursing team, the Memory Care Coordinator provides overall coordination in developing the memory care service line.   The Coordinator provides assistance in planning, developing, organizing, implementing, evaluating, and directing the Memory Care teams to assure excellence in person centered resident care, activities programming, and customer service.    The Memory Care Coordinator will ensure resident programs and services meet company standards and expectations.  Traveling is required.


 


Job Duties



  • Assures that all residents and their families are treated with dignity and respect.

  • Understands, demonstrates, and promotes the Primrose Mission Statement and Core Values.

  • Provides clear direction and goals to the Memory Care teams in each community to meet company standards and expectations.

  • Collaborates with Regional Teams (Operations Managers, Nursing Managers, and Sales Managers) to achieve clinical, sales, and operational benchmarks.

  • Provides regular updates/status reports to the Executive Director of Nursing.

  • Emphasizes positive family communications and relationships by all Memory Care teams.

  • Facilitates optimization of software programs, nurse call systems, and other technology.

  • Provides teaching, training, and auditing to assure staff education and competencies are completed.

  • Assures regulatory compliance for memory care services in all States.

  • Works closely with the Training and Development Coordinator to implement training programs.

  • Mentors staff in solving resident issues and concerns as well as managing behavioral expressions.

  • Facilitates regular (daily) meetings of each memory care team to review all aspects of resident care.

  • Works closely with the Dining Service Specialist to assure the dining experience as outlined in the Primrose Program Guide is accomplished.

  • Works closely with the Life Enrichment Specialist to assure the development of person-centered activities programs that incorporate the Life History of each individual resident.

  • Assists in selection and retention of high quality staff.

  • Assists clinical staff in assessing prospective residents prior to move-in to determine the appropriateness of the community according to the resident’s level of care.

  • Coordinates community education topics with all Memory Care teams.

  • Assists in developing education programs for Assisted Living staff in communities where Memory Care units are added to the campus.

  • Performs any other related duties as requested by the Executive Director of Nursing.

  • Spends adequate field time to mentor, develop and maintain Primrose Memory Care standards and drive results.

  • Works with Sales and Marketing team to share inspiring stories.

  • Travel is expected 25% to 50% of the time.


 


The Ideal Candidate will have:



  • Four (4) year college degree or equivalent from an accredited college or university.

  • Minimum of three (3) to five (5) years of experience in memory care within a senior living or long-term care environment.

  • Knowledge of Alzheimer’s Disease, and other types of dementia, as well as other related memory impairments required.

  • Experience with person-centered care philosophy and techniques for addressing challenging behavioral expressions preferred.

  • Previous management experience required; multi facility management preferred.

  • Experience as a Recreational Therapist or Occupational Therapist in senior care desired.

  • Ability to speak effectively before groups of residents, associates of the organization, and/or outside professionals.
