Job Description:

Clinic Social WorkerBeatrice Family & Internal Medicine Full-Time; Monday-Friday 8:00a-5:00p. Provide professional social work services for patients and families of the clinics through a diverse role as a educator, care coordinator, patient advisor and advocate. Conduct psychosocial assessments, develops a plan of care through the use of social work practice, theory, and knowledge for all ages according to established policies, procedures and guidelines and coordinates care across the medical and community service resources. Make appropriate referrals. Work in a collaborative relationship with other healthcare team members. Address barriers to care and social determinants. Identify social hardships within the community and population served. May also provide case management interventions to promote healthy positive outcomes. Oversight provided by a Masters of Social Work. Bachelors Degree in Social Work, Masters Degree in Social Work preferred. Current licensed as a social worker in the State of Nebraska. Current CPR/BLS. Outpatient clinical experience preferred. 1-2 years experience preferred.
