Position Summary

Provide health care services to patients who may have acute episodic conditions or chronic problems, as well as those seeking health evaluation, preventive care and/or education and counseling.

The position works closely with Lead Clinician and Clinic Manager to ensure comprehensive delivery of services and quality patient care.

Job Requirements for APP Applicants

• Master's Degree from an accredited Nurse Practitioner Program or PA training program
• National Provider Identification Number (NPI).
• Two (2) years’ experience as a Family Medicine Nurse Practitioner.
• Current and unrestricted California NP/PA License
• Board of Registered Nurses Certification Furnishing Number issued by California Board of Registered Nursing
• Current and unrestricted Controlled Substances Registration Certificate from the U.S. Drug Enforcement Administration (DEA)
• Valid California Driver’s License, proof of auto liability insurance and clean driving record
• Current Basic Life Support (BLS) Certification for Healthcare Providers

For Physician Applicants

• Completion of an accredited US residency program in Family Medicine or Internal Medicine
• Board Certified/Eligible
• Clear, active CA medical license

Please keep in mind that the range mentioned above is the full base salary range for the role.
