Highland Rivers Behavioral Health provides compassionate, professional treatment and recovery services to adults, children, families and veterans affected by mental health issues, substance abuse and addiction, and intellectual and developmental disabilities. JOB SUMMARY: The WTRS Peer Specialist models recovery by using lived experience as a tool for the service intervention within the scope of their knowledge, skills, and education. The WTRS Peer Specialist provides program participants with peer support services in individual and group settings, serves as liaison between program and referred individuals to ensure appropriate access to services, and promotes a culture of recovery within the organization. Seeking to contribute to the success of the WTRS program, as well as the agency as a whole, the WTRS Peer Specialist successfully carries out daily tasks including facilitation and documentation of group sessions, individual peer support sessions, completion of WRAP plans, crisis intervention, resource development and linkage, and evaluation of participants' progress, through acquired knowledge, skills, and abilities.

DUTIES ANDRESPONSIBILITIES:


  • Provides peer support services in individual and group settings, as outlined in DBHDD Provider Manual. Documents in accordance with agency guidelines to monitor individuals' progress toward their individualized recovery plans.

  • Serves as liaison between program and referred individuals to promote appropriate access to services and increase retention, in accordance with program capacity requirements.

  • Assists with coordinating entry to program, including initial screening processes, weekly phone contacts, and orienting individuals to program upon admission.

  • Facilitates group training sessions including but not limited to peer support, psycho-education, didactic, and skills groups. Completes daily documentation of group sessions in EMR system, using BIRP format, in accordance with agency same day documentation policy.

  • Assists in providing interim services by phone to individuals identified as priority status when residential placement is not available.

  • Provides structured activities that promote socialization, recovery, wellness, self-advocacy, development of natural supports, and maintenance of community living skills.

  • Attends and participates in case staffing and treatment team meetings as directed by direct supervisor.

  • Collaborates with a multi-disciplinary team in reviewing cases and developing individualized recovery plans, including referrals to case management, advocacy groups, skills training groups, social rehabilitation and treatment providers.

  • Provides consultation to referral sources, community stakeholders, and members of individuals' support system to help support individuals in their recovery process.

  • Works cooperatively with the legal system to ensure coordination of services including court appearances when appropriate.

  • Intervenes and manages acute crisis situations as needed.

  • Completes in person and online training to encourage professional development and meet agency, accreditation and regulatory standards, as scheduled.

  • Other responsibilities and tasks as assigned.
