Case Coordinator, Community Support Services at HUMANIM INC in Columbia, Maryland

Job Description:

Reporting to the Program Manager, this role will be responsible for the full case management of the assigned caseload and coordination of appropriate services for clients. This position provides individualized teaching, training, and support needed to adults with severe and persistent mental illnesses to help them maintain their independence in the community. Case Coordinators will work with clients in their homes and throughout the community to provide life skills training and case management services.
This role assumes responsibility for advocating for, coordinating the delivery of, and monitoring a wide variety of services and supports for eligible persons to enable people to lead meaningful and productive lives. Additionally, you will be working as part of an interdisciplinary team with opportunities for cross-training, development, and clinical supervision with the option of becoming a Certified Psychiatric Rehabilitation Practitioner (CPRP). This role allows for a flexible schedule.
