Transport Coordinator (EMT/Paramedic/LVN) - Austin, TX at Texas Children's Hospital in Austin, Texas

Job Description:

To facilitate patient transfers to/from outside healthcare facilities (Transfer Center). To place patients within Texas Children's Hospital (Bed Control) in order to maximize available beds to patients requesting beds based on acuity, diagnosis, age and sex. This is accomplished by cross training in both areas to ensure a working knowledge of both the Transfer Center and Bed Control functions. Room Management works to ensure that Bed Placement Guidelines and state and federal regulations are adhered to.

Licenses and Certifications


  1. LVN - Lic-Licensed Vocational Nurses Texas Board of Nursing or Nurse Licensure Compact Or X EMT-B - Emergency Medical Techn-Basic Texas Department of State Health Services

  2. H.S. Diploma or GED

  3. 2 Years of Transfer Center and/or 911 dispatch experience

Responsibilities:


  • Constantly monitors the bed request tab in Epic in order to place patients in an appropriate room and bed. The bed placement must be assigned and approved in a timely manner.

  • Collaborates with Nursing House Supervisors to secure specific clinical information to make appropriate bed placement decisions.

  • Ensures patients who require isolation are placed in the appropriate room as ordered in EPIC or as directed by Nursing House Supervisors or Infection Control.

  • Evaluates Main Admissions and Same Day Surgery schedules in making clinical assessments in order to make timely and appropriate room assignments for patients.

  • Notifies families of bed availability for Main Admissions

  • Consistently adjusts preassigned patient assignments to available open beds.

  • Facilitates the transfer of incoming and outgoing patients by connecting clinical staff of sending and accepting facilities to determine the medical appropriateness of transfer. All transfers are cleared and approved for clinical appropriateness with accepting physician without exception, unless extenuating documented circumstances exist and are approved by Leadership.

  • Follows all workflows and documents the transfer of incoming and outgoing patients by collecting required information on the transfer request form within federal/state regulatory requirements and Hospital policies. All transfers have complete documentation of required data elements, Transfer extensions are obtained when processing time exceeds 30 minutes without exception, unless extenuating documented circumstances exist and are approved by leadership.

  • Coordinates the exchange of medical records and imaging

  • Dispatches the transport team as directed by the Kangaroo Crew Charge Nurse in a timely manner

  • Communicates all transfer dispositions and follows the appropriate workflows for acceptance and denials. Contacts room management leadership

  • Facilitates the transfer of incoming and outgoing patients by connecting clinical staff of sending and accepting facilities to determine the medical appropriateness of transfer.
