LPN/RN MDS NURSE RESPONSIBILITES

• Collaborate with campus nurses to complete MDS assessments, care plans and CAAs


• LPN/RN MDS Nurse will work with various disciplines to balance the need to maximize RUGS/Medicaid case mix with Quality Measures as they relate to the 5-star program


• Thorough understanding of PDPM and assist in transitioning to this model


• Develop concise and individualized care plans and coordinate with other disciplines while being an excellent problem-solver/communicator


• LPN/RN MDS Nurse will monitor and manage Quality Measures and complete related assessments


• Assess and implement QAPI processes and projects


• Collaborate with the interdisciplinary team as needed in plan of care meetings, utilization review meetings, chart reviews and any other meetings and projects

MDS NURSE REQUIREMENTS

• Valid LPN or RN license in the State of Ohio


• Preferred prior experience as an LPN/RN MDS Nurse
