UC Health is hiring a full-time License Practical Nurse (Supervisor - Clinical LPN) for Bridgeway Pointe for the first shift at Daniel Drake Center for Post-Acute Care.
The Bridgeway Pointe Nurse will support Daniel Drake's Bridgeway Pointe delivering high-quality care to patients based on assessed needs, established standards of care and according to policy and procedures. This unit has a total of 72 residents within our assisted living facility and has other beds for memory care. Our nurses help pass medications to these residents and provide the best care to those residents within our facility.

• Engages in population appropriate communication.
• Has knowledge of growth and development milestones and tasks.
• Gives clear instructions to patients/family regarding treatment.
• Involves family/guardian in the assessment, initial treatment and continuing care of the patient.
• Identifies any physical limitations of the patient and deploys intervention when necessary.
• Recognizes and responds appropriately to patients/families with behavioral health problems.
• Interprets population related data and plans care appropriately.
• Identifies and responds appropriately to different needs resulting from, unique psychological needs or those associated with religious / cultural norms. Performs treatments, administers medication or operates equipment safely.
• Recognizes and responds to signs/symptoms of abuse or neglect.
Leadership:
• Supervises staff assigned to their unit or center of excellence and demonstrates the ability to coach and mentor staff. Operational leadership of shift including patient flow and assignment/adjustment of resources to meet dynamic changing needs throughout the shift.
• Champion efficient throughput and capacity management.
• Benchmark internal processes to improve practice.
• Lead and implement operational improvement initiatives and assures compliance via staff coaching and mentoring.
• Ensures staff behaviors supports and are consistent with the PRIIDE Values, Missions and Vision of UC Health.
• Provide transformational leadership as related to PFCC.
• Assist with developing and revising of departmental standards, policies and procedures.
• Staffing and scheduling responsibility for cohort of staff and shift.
• Promote patient care safety standards.
• Actively identifying opportunities for improvement in patient satisfaction, perform trials and report back to leadership on results.
• Facilitate conflict resolution.
• Empower staff to participate in decision making through shared governance/UBC.
• Maintain, support and encourage others.
Clinical Resource and Competency:
o Maintain hospital wide and department specific nursing competencies and completes annual competencies in required time frame.
o Collaborate with the Department Educator and/or provides for orientation and educational needs identified in the department.
o Function as a clinical resource for area of clinical specialty.
o Promote the use of evidence-based practice.
o Assist in identifying and operationalizing best practices in the unit.
