We utilize a team-based model including embedded Behavioral Health Clinicians as well as Case Managers, Care Coordinators and Community Health Workers.

The Manager of Social Services supervises the integrated case management team at Amoskeag Health, a community health center that serves a diverse patient population and acts as a primary safety-net provider for the Greater Manchester area. The case managers support our medical and behavioral health providers in providing the highest quality, most appropriate care while promoting the cost-effective utilization of organizational and community resources. The person in this management role is responsible for supervising, supporting, and monitoring a structured, evidence-based model of social service delivery in which Social Determinants of Health are addressed, progress of individuals is centrally tracked, and the health of vulnerable and underserved populations is improved.
