SCOPE OF ROLE


Reporting to the Director, Pharmacy, the Associate Director, Clinical Pharmacy assists in fulfilling Signature Health’s mission and vision by providing clinical expertise, leadership, oversight, and direction for Signature Health’s clinical pharmacy services. The Associate Director, Clinical Pharmacy will oversee the ongoing development, implementation, and compliance of all programs relating to clinical pharmacy.  The Associate Director, Clinical Pharmacy will support business and patient care operations at all Signature Health locations through optimization of workflows, development of staff, and commitment to positive patient outcomes.


HOW YOU’LL SUCCEED:



  • Prioritize, develop, and implement new clinical pharmacy programs, including medication therapy management, medication reconciliation, and collaborative practice agreements.

  • Actively engage with each team member’s individual performance through ongoing coaching, development, and meeting objectives through the annual review process.

  • Able to perform all job functions of a Pharmacist and provide coverage if needed.

  • Build relationships by providing comprehensive medication education to patients; understand the patient’s health literacy status when offering intervention and teach-back methods in order to increase medication adherence.

  • Provide appropriate pharmacotherapy consultation through the review of patient medications, updating medication profiles, and discontinuing medications when appropriate, as well as reviewing all available sources to maintain a complete and accurate patient record.

  • Collaborate with members of patient care teams and Signature Health clinical leadership to provide therapeutic recommendations and implement practices to maximize patient access, adherence, and patient outcomes.

  • Serve as clinical pharmacy point of escalation for inquiries requiring a higher degree of expertise or discretion to resolve concerns.

  • Identify improvement opportunities and offer solutions to drive growth, improve efficiencies, optimize medication therapies, and advance patient care.

  • Actively develop, participate, and lead team meetings and support training opportunities to improve outcomes and patient care as well as further the educational development of pharmacy professionals. 

  • Maintain and update clinical and professional knowledge and skills through formal and informal educational experiences to ensure best evidenced based practice. 

  • Adhere to all Signature Health policies and procedures, as well as state and federal laws.

  • May be required to perform remote services as required by Signature Health.

  • Other duties as assigned.


KNOWLEDGE & EXPERIENCE:



  • Valid unencumbered Registered Pharmacist (RPh) License in the state of Ohio required.

  • Bachelor of Science degree in Pharmacy from a university accredited by the American Association of Colleges of Pharmacy required; graduate degree such as MS, MBA, PharmD strongly preferred.

  • Minimum 5 years of recent pharmacy experience in clinical setting required; PGY1 residency completion strongly preferred.

  • Minimum 3 years of pharmacy supervisory and administrative experience required; prefer minimum 3 years of experience developing and implementing clinical pharmacy services.

  • American Heart Association (AHA) Basic Life Support (BLS) certification required.

  • Demonstrated proficiency in basic computer skills (i.e., Microsoft Windows, Outlook, Teams, Word, PowerPoint, Excel).


WORKING CONDITIONS:



  • Work is normally performed in a typical interior/office/clinical work environment.

  • While hours of operation are generally standard, flexibility to work evenings and extended hours may be required.

  • Requires periods of sitting, standing, telephone, and computer work. 

  • Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology.

  • Speaking: adequate to speak to clients or patients in person, over the telephone or through telehealth technology.

  • Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.

  • Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted.

  • Possible exposure to blood borne pathogens while performing job duties.

  • Frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular workday.

  • Sufficient dexterity to operate a PC and other office equipment.
