Our Population Health + Wellness team is seeking a key leader to join our growing, elite group tasked to drive progressive thinking in how we understand the health risks within employer groups, measure the effectiveness of innovative solutions and make tactical and strategic recommendations on business issues. We partner with our clients to understand their strategy and deliver unique solutions to address the risks within their population.
The Population Health + Wellness Strategist will also be tasked in further developing and outseeing the population health deliverable. This includes collaborating with our analytics team to build out dashboards that identify at-risk individuals within an employer population. This role will also serve as an accountability partner to third-party wellness/clinic provider(s) to ensure expectations of the solution are being reached.
JOB DESCRIPTION
Bring a consultative approach to clients on strategic initiatives for employer wellness and population health solutions
Build and manage direct client relationships on behalf of the Population Health and Wellness deliverable
Understand wellness and condition management vendor market and be able to convey to clients' what options are available to fit specific needs
Support clients through implementation process and ongoing needs of wellness and condition management solutions
Collaborate with third party wellness vendors to review clinically at-risk populations, make individual action plan, and have continual engagement with wellness vendor on progress of action plan
Understand employer's health plan resources for additional support to third party solutions
Engage and manage the Johns Hopkins ACG outputs with the help of the analytics team
Become the subject matter expert (SME) on the Johns Hopkins ACG tool, and expanding our current population health analytics deliverable
Review and grow population health reporting with Population Health + Wellness team
Engage in health and wellness market to bring unique solutions to clients for targeted care
TECHNICAL FUNCTIONS
Proficiency in MS Office products (Excel, Word, Outlook, PowerPoint)
Demonstrate proficiency with data analysis and ability to interpret data to recommend third party solutions
Ideal candidate would have knowledge with clinical risk scoring algorithm (Johns Hopkins ACG, MEDai, etc.)
REQUIRED EXPERIENCE
2 + years in wellness related role
Excellent oral and written communications skills
Knowledge and ability to assess barriers to care and ability to identify solutions or modifications to ensure successful outcomes. i.e. identifying resources and necessary support
Demonstrate ability to be self-directed, highly organized and multi-tasked capable
Proficient in problem solving skills and critical thinking
Outstanding customer service skills and ability to adapt approach to various situations
Consulting background and/or health insurance/ benefits background (preferred)
