About the Program and Position:
The ACS Program supports foster youth in the custody of the Administration for Children's Services in New York City. Our Enhanced Family Foster Care (EFFC) program serves 137 youth ages 0-21+ and supports them with a multidisciplinary team that includes a Case Planner, Health Services Team, Psychiatrist, Behavioral Support Specialist, Family Advocate, Case Aide, Family Team Conferencing Coordinator, Education Workforce Specialist, Employment/Internship and Housing Specialist, Fair Futures Coach, Foster Home Recruiter and Licensor and Clinician. Our therapeutic approach to foster care includes direct access to Clinicians and foster parents that are trained at a treatment foster care level implementing the ARC framework. This makes a difference because our team approach provides a space for healing while providing skill-building and future-focused planning for youth in our care.
Cayuga Centers is looking to hire a passionate and dedicated Case Planner for the NYC Enhanced Family Foster Care (EFFC) program to coordinate quality care and support to foster youth. The Case Planner collaborates with foster youth, their families, and foster parents to develop and manage a permanency plan that ensures the child's safety and well-being through regular contact and home visits.
How you get to use your skills...
Complete monthly in-person home visits with youth, parents, and foster parents to ensure safety, wellness, and permanency
Ensure that children are safe and well cared for in the foster home through the development and maintenance of a positive working relationship with the child and foster parents.
Maintain regular contact with child/foster parent and immediately respond to early signs of disruption.
Gauge parents' progress in achieving service goals through careful observation of parent/child interaction at visits, feedback from other service providers, home visits, and interviews.
Continuously explore and identify kinship resources for all youth.
Ensure that each child is progressing toward permanency with the goal of achieving permanency within a year.
Work collaboratively and communicate clearly and respectfully with colleagues, families, and other service providers.
Ensure thorough and timely written and verbal communication with families, colleagues, ACS, OCFS, and other stakeholders on case developments
Supervise family/sibling visits both at the agency and off-site.
Write timely, clear, concise progress notes, FASPs, Permanency Hearing Reports, and other documents.
Attend weekly supervision meetings to report on cases and present for discussion any problems, difficulties, or unresolved issues.
Participate in staff meetings, seminars, and training to maintain and apply knowledge of internal and external case planning procedures and policies applicable to foster care.
Attend and & play an active role in Family Team Conferences (FTC's), Interdisciplinary Treatment Team (ITT) meetings, and Parent to Parent Meetings
Appear in Family Court hearings related to the children and families served
Respond to client emergencies within or outside work hours when on-call
Assist with hospitalization and discharge planning following hospitalization
Maintain progress notes and manage the case record in NETSMART and Connections
Perform other related duties as requested by the immediate supervisor or the CEO
Schedule: Non-Exempt
Regular hours entail 9:00 am - 5:00 pm, Monday through Friday
Must be able to flex schedule
On-Call required

Familiarity with OCFS, OPWDD, OMH, and other relevant municipal and state agencies

Valid NYS Driver's License preferred
Physical Requirements:
Word processing: Including responding to e-mails, composing letters, and transcribing minutes for meetings, involves sitting at a computer for several hours a day
Completing paperwork and electronic documentation
Must be able to lift up to 50 pounds
Sit/ride in vehicles and/or public transportation for lengthy periods of time
Walk up/down stairs
Walk several blocks to get to the required locations
