Job Title:Specialty Practice Pharmacist


Department:East | Outpatient Care East Infusion Pharmacy



Job Description SCOPE OF POSITION: Ensure compliance and accuracy of medication use process across OSU and OSUPhysicians ambulatory clinics and OSU ambulatory surgery centers. In cooperation with other pharmacists and pharmacy technicians, acts as a resource and provides answers to drug information questions for patients, family, pharmacists, nurses, physicians, medical students, pharmacy students and other associate hospital personnel. Assists with infusion and ambulatory OR staffing. Approves new drug orders, which may include checking/verifying chemotherapy notes, informed consent and processing non-formulary requests. Screens for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food (in cooperation with the Dietetics Department), drug-disease state and drug-laboratory interactions. Participates in appropriate ambulatory, surgical and/or departmental committees relevant to practice area. POSITION SUMMARY This position functions to coordinate all aspects of medication use in the ambulatory patient care area and ambulatory OR areas. Duties include but are not limited to: review medication storage, ordering, administration; and ensure compliance with Ohio State Board of Pharmacy Rules and regulations and compliance with Ohio State policy and procedures. Review of ambulatory medication safety events. Patient population includes adult, adolescent, geriatric, and neonatal patients depending on the practice location. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must be able to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age. This position is also responsible for supervising Pharm.D. students, technicians, undergraduate and graduate student teaching, and providing educational programs for hospital staff (physicians, nurses and pharmacy staff). Minimum Education Required Bachelor's Level Degree Additional Education Desired Master's Degree Required Qualifications Registered pharmacist in the State of Ohio; board certified; advanced degree (Pharm.D./MS) and residency; and/or equivalent experience preferred (e.g. PGY1 and PGY2 residency).
