Job Title: Home Health Physical Therapist



Duties & Responsibilities: Home Health PT sought for 13 week assignment with the potential to extend. Schedule would be 5x8 day shifts Monday-Frday. The coverage area is Centerville and surrounding areas (roughly 7 counties but will be scheduled accordingly so visits are not widely spread each day).



Minimum Requirements:

• BLS


• Home health experience required


• HCHB experience preferred
