Physical Therapist Schedule:




  • 8:30am to 5:00pm Monday through Friday


Objective: To provide ongoing therapy evaluations and treatments and education to residents in a
timely manner per physician's orders. To provide leadership and supervision of physical therapy services to
other physical therapists, assistants and aides. The Physical Therapist evaluates and treats patients,
communicates with families, physicians and other health team members and maintains appropriate
documentation of services including MDS responsibilities complying with State and Federal regulations.
Participates in on-going facility meetings and conducts in-service and training for facility staff on ongoing
basis in restorative and maintenance techniques.



Required Qualifications: To perform this job successfully, an individual must be able to perform each

1. Graduate from a physical therapy curriculum approved by the American Physical Therapy Association
and is currently licensed as a Physical Therapist in state where practicing.
2. Able to understand medical terminology and how it applies to physical therapy, to communicate with
residents, staff, physicians, and to prepare meaningful reports.
3. Spatial and form perception are necessary to visualize anatomical structure of the disabled part of the
body and the relation to therapeutic devices in remedial actions.
4. Coordination and dexterity are necessary when treating residents and handling therapeutic equipment.
5. Ability to communicate with residents to establish favorable attitude and to motivate the residents to
desired cooperation and effort in his/her rehabilitation development.
6. Possesses adequate strength and endurance to meet the physical demands involved in moving residents
and equipment into position for treatment and in manipulating equipment and residents.
7. Be willing to work weekends and holidays, as needed, on a rotational schedule.
8. Be willing to travel and work at other VOA facilities, as needed.

9. If operating company vehicle you must be twenty-one (21) years of age and meet the MVR organization
standards.
10. Maintain productivity standards as directed by supervisor.
11. Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and
documentation.



Essential Functions: To perform this job successfully, an individual must be able to perform

1. Accurately screens and evaluates residents and provides PT upon written order of physician within
twenty-four (24) hours of referral.
a. Evaluates each resident and initiates any ordered treatment(s).
b. Maintains a line of communication with appropriate agencies for prior and continuing
authorization of therapy treatment.
c. Communicates a report regarding the residents' progress to attending physician.
d. Consults with the physician regarding residents' change of condition as indicated during the course
of treatment.
e. Re-evaluates the resident in conjunction with the attending physician.
f. Supervises the operation of physical therapy services to ensure an acceptable level of performance
from assistants and support staff.
2. Prepares and maintains treatment records with evaluation and progress notes; including:
a. Physician's orders.
b. Objectives of treatments.
c. Plan of care.
d. Treatment procedure(s) and baseline clinical data.
e. Evaluation of progress and resident's response to treatment.
f. Other pertinent information as required by facility's policies.
3. Supervise physical therapy assistants and/or physical therapy aides in direct resident care and residentrelated activities according to State and Federal requirements. Counter-signs documentations per
regulations.
4. Participates in resident care planning conferences, initial conferences, for residents on physical therapy
programs, and when requested by nursing and/or therapy department.
5. Participates in other facility meetings as directed by Director of Rehabilitation.
6. Communicate with Director of Rehabilitation and other health team members regarding resident
progress, problems and plans for specific residents.
7. Confers with Director of Rehabilitation on an ongoing basis relevant to the development and
implementation of department objectives with respect to overall facility and organizational goals.
8. Participates in PPS team process monitoring minutes of therapy and understanding the PPS systems.
9. Maintain productivity standards as directed by supervisor.
10. Assists therapy assistants in maintaining productivity standards.
11. Maintains proper documentation and billing functions using computer.
12. Assists with personnel functions of the Physical Therapy Department along with Director of
Rehabilitation:
a. Interviews and hires department personnel; supervises orientation; counsels and/or disciplines
employees on infractions of established rules; terminates employees when needed; conducts exit
interviews of department personnel.
b. Conducts performance evaluations and recommends wage changes.
c. Develops, implements, and maintains work routines and staffing schedules.
d. Initiates and directs in-service training programs for the department.
13. Participate in the MDS process as a member of the interdisciplinary team.
14. Participate in the PPS team process monitoring minutes of therapy and understanding the PPS systems.
15. Confers with the Director of Rehabilitation on an annual basis in the development and implementation
of department objectives with respect to overall facility and organizational goals.
16. Teaches appropriate physical therapy procedures and supportive activities to residents, family members
(as needed), and other health care personnel who share responsibility for residents care in a variety of
settings.
17. Teaches principles of correct body mechanics to all employees as scheduled by the In-Service Director.
18. Makes rounds with designated nursing personnel on all residents receiving physical therapy at regular
intervals.
19. Advises, secures and helps maintain durable medical equipment for therapy department and residents.
20. Assists in ensuring that physical therapy services are provided on a continuous basis without
interruption. This includes finding supportive personnel if regular employee is absent due to vacation,
sickness, leave of absence, or other reason. Assists with finding replacement for therapist/assistant
when needed.
21. Supervises maintenance of equipment and supplies, and evaluates and notifies Director of
Rehabilitation, on a continuing basis, of the condition of therapy equipment.
22. Attends required in-services & completes assigned online modules
23. Comply with the Physical Therapy Practice Act in the state.
24. Performs other duties or special assignments as directed by the Director of Rehabilitation
