Location: Fayetteville, NC

Description:
Our client is currently seeking a Nurse Practitioner near Fayetteville, NC to help cover 4 skilled nursing facilities.


Scope of Role:


# of facilities to visit: 3-4


Contract Length: 4 to 7 months


Hours: M-F 8am to 5pm


Start Date: June 2024


Hours could range from 20-32 per week based on needs. Needs also to support staffing coverage for PTO and FMLA and growth.

This Job Will Have The Following Responsibilities:




  • Conduct comprehensive assessments, patient facing Effectively manage acute and chronic medical and behavioral conditions

  • Ensure accurate and complete documentation, including ICD 10 conditions

  • Communicate and collaborate with the interdisciplinary care team

  • Conduct advanced illness and advanced care planning conversations Provide patients and caregivers with counseling and education



Qualifications & Requirements:




  • NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program

  • Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following:



Family Nurse Practitioner Adult


Nurse Practitioner Gerontology


Nurse Practitioner Adult-Gerontology


Acute Care Nurse Practitioner




  • 1+ year of clinical experience in practice (long-term care setting preferred)
