We all have our own vision for the way we want to live our lives. For a person with an intellectual or developmental disability, that vision is documented in their Person-Centered Plan (PCP).
As a Support Coordination Manger, you will manage and facilitate the person-centered planning and related activities for assigned department(s). The Support Coordination Manager will train and coach Support Coordinators in the assigned department(s) with a goal of supporting persons served to achieve their vision for themselves and support team members at every level in the facilitation of this empowerment.
This position is a cross organization, collaborative, systems focused position to support the assigned department(s) in ensuring compliance with person-centered planning, stakeholder feedback and personal outcomes of those served. The Support Coordination Manager will be responsible for working closely with internal PCP Facilitators, Coordinators of Community Services and the Developmental Disabilities Administration.
This position requires a high school diploma or GED with at least three years of professional experience providing person centered planning for people with intellectual and developmental disabilities, though a bachelor's in social work or a related field is a plus. Candidates must also possess a valid driver's license with no egregious records and complete all required Arc and Developmental Disabilities Administration (DDA) trainings, becoming trained as a Certified Medication Technician.
