Job Description:

SIGN ON BONUS
The Resident Care Coordinator (RCC) is a position whose main function is to oversee the medication management program and support the Resident Care Director (RCD) in the day-to-day assisted living wellness operations.
Major Duties and Responsibilities:
Staff Management, Supervision and Training

Supervises Med Techs to ensure safe administration of medications in accordance with federal and state guidelines and in adherence to the policies, procedures, and guidelines of the facility.
Interviews and recommends hires of new Med Techs.
Schedules Med Techs and always ensures appropriate staffing levels.
Works as a Med Tech for one main med pass weekly and during staffing shortages.
Conducts performance reviews and completes performance feedback designed to enhance employee work performance.
Provides training, feedback, and work direction to Med Techs.
Develops and provides ongoing in-service training and retraining to new and current Med Techs as a group or individually.
Identifies staff performance issues and recommends supervisory or disciplinary action.
Monitors Med Techs for compliance and ensures adherence to the policies, procedures, and guidelines of the facility required by Med Techs.
Ensures that personal care assigned to the Med Techs is completed and documented.
Alerts Med Techs to instances requiring 72-hour charting and monitors compliance.

Incident reports.
Antibiotics & new medications.
Residents who have returned from hospital stays.
New Admissions.

Medication Acquisition, Documentation and Department Management

Checks for pharmacy deliveries and that medications have been checked in and are on the correct carts.
Reviews and sends orders to pharmacy, following up as necessary until medication is acquired.
Approves medications in Quick Mar
Monitors Quick Mar throughout the day identifying instances of med variances and works with Med Techs to minimize variances.
Review all new resident's physician's orders and ensure that medication is available upon move in.
Ensure physicians' orders for all residents are updated every six months, minimally.
Assesses all new residents who desire to self-medicate and performs reassessment every six months.
Plans, coordinates, and adjusts medication schedules to ensure that all residents receive medication and that other physicians' orders are followed in a timely manner.
Ensures weights & blood pressures are kept current daily/monthly as ordered.
Weekly audits of medication carts/equipment in coordination with the RCD and MCD.
Meds are organized.
Meds are labeled and dated.
Narc book is correct with no missing medications or missing signatures.
Communicates any missing medications, med errors and other anomalies to RCD.
Refrigerator temps are logged and adjusted as necessary.
Monitors medication overflow room, and ensures it is organized and clean.
Routinely oversees destruction of medications and ensures documentation is completed in coordination with RCD.
Ensures that medications are stored correctly.
Monitors Narcotics Administration Record and ensures that controlled substances are signed out and accounted for by Med Techs.
Identifies the need for supplies and/or equipment necessary for Med Techs to perform duties and advises RCD of needs.
Ensures that resident charts are being maintained and audits 5 charts weekly for accuracy and completeness.
Reviews Friday Numbers report prepared by Regional Director of Clinical Support and prepares explanation for any variances.
Maintains and prepares documentation and reports as requested.
Participates in the Manager on Duty rotation.
Attends A.M. Stand Up Meeting
Other duties as assigned by RCD or ED.
