Valor Healthcare is looking for a passionate Mental Health Nurse Practitioner to join our team at the Community Based Outpatient Clinic (CBOC) in Fort Pierce, FL. Position Summary

Diagnoses and treats mental and behavioral health conditions. This includes diagnosis, evaluation and management of psychosocial dysfunction. The mental health nurse practitioner completes assessments, treatment planning, short- term treatment, long-term treatment, education, as well as prescription and management of necessary mental health related medication. This includes appropriate follow up on lab, medical condition and mental health stability. The mental health nurse practitioner provides referral for resources and benefits as needed by the veteran. Collaborates with the core PACT Team (Primary Care Provider, RN and Medical Assistant) and expanded PACT Team including family/caregiver, VA, and community-based services involved in developing the patient care plan. The mental health nurse practitioner works collaboratively with their supervising physician following individual state licensing requirements.



Schedule

This position is 32 hour a week during our clinic hours of Monday through Friday, 8 - 4:30 PM ET. Ideally the schedule will be 8 hour days on Monday, Tuesday, Thursday and Friday. Although, we will consider alternative schedules that fits the provider's needs within clinic capabilities.


This position is not remote eligible.

Core Responsibilities

  • Evaluates veterans for mental health disorders and appropriately diagnoses and treats mental health conditions.
  • Orders and performs diagnostic tests and therapeutic procedures as related to mental health diagnoses and treatment and interprets test results for deviations from normal and provides follow up care.
  • Prescribes medication and recommends dietary and activity programs.
  • Performs appropriate interval follow up visits for necessary treatment of mental health diagnosis.
  • Refers patient to the VA medical specialist when indicated.• Counsel patients on the use of prescription medications and educates patients in relation to treatment plan.
  • Develops and implements patient management plans, assists in provision of continuity of care through the PCMM (Primary Care Management Module).
  • Evaluates patients records from medical providers outside the VA and works with the veterans utilizing rules set forth by the VA for co-managed care.
  • Conducts individual counseling/therapy.
  • Contributes to group therapy (i.e., PTSD groups; weight management; smoking cessation; grieving; and personal interactions).
  • Contributes to pain management plan as needed.
  • Involves veterans in planning and assuming responsibility for activities.
  • Refers veterans, when indicated, to community resources and other organizations.
  • Addresses issues such as advance directives and organ donation in accordance with acceptable standards.
  • Inputs all patients' information into VISTA/CPRS.
  • Assists in co-managed care collaboration with non-VA providers, and facilities.
  • Maintains confidentiality of veterans' data and information.
  • Complies with all VA and company training requirements.
  • Fulfill compliance requirements of the Office of Inspector General (OIG), Joint Commission(JC), Environment of Care (EOC) oversight, lab compliance and other related items.
  • Remain focused on achieving excellent clinical outcomes through the specified VA guidelines.
  • Must provide excellent customer service to each veteran and his/her family, both in person and over the phone, as well as to fellow colleagues and clinic visitors.
  • Participate in the clinic's outreach events to help support the clinic's enrollment initiatives.
  • Support patient enrollment and retention in appointments, follow-up visits or nurse visits.
  • Embrace and support new initiatives, whether clinical or operational
Requirements:

Qualifications

  • Graduate of an accredited school of nursing and graduate of an accredited program for nurse practitioner (MSN required), including preceptorship
  • Certification by applicable professional organization
  • Minimum three-year clinical experience as an adult mental health CRNP. (Specific requirements could vary based on individual VA contract).
  • Must be credentialed and remain in good standing through the Veterans Health Administration (VA).
  • Must comply with and maintain all requirements for a valid, unrestricted license in the state of desired employment, or in any U.S. state or territory, depending on VA contractual requirements.
  • Current certification in Basic Life Support (must be renewed periodically as specified by the certifying agency – AHA valid for two years, e.g.) and in ACLS as specified by individual VAMC contract
  • Current, unrestricted Drug Enforcement Administration (DEA) registration
  • Proficiency in written and spoken English.

  • This is considered a safety sensitive position.
