SOCIAL WORKER/ FULL TIME/ 40 HOURS/ LICSW SICKLE CELL TEAM

Job Summary:




  • Join a close-knit committed team of clinicians within the Comprehensive Sickle Cell Disease Treatment Center focused on caring for people with Sickle Cell Disease (SCD). Partner with another dedicated SCD social worker.


  • Provide psychosocial care for patients and families including assessments and interventions. Ongoing follow up is a vitally important element of the work, and the Social Worker will build lasting long term relationships with patients across the continuum of care, including survivorship.


  • Engage in program development and expansion such as creating support groups and other initiatives.


  • Participate in developing new care models.


  • Offer educational consultation for other disciplines across the hospital to share knowledge of Sickle Cell Disease including addressing implicit bias, dispelling misconceptions, and informing how racism can detrimentally affect patient care.


  • Help patients facing challenges transitioning from pediatric/ adolescent care into adult care.


  • Work schedule may include some remote work since Social worker will see patients both inpatient and in follow up after leaving.

Sickle cell disease (SCD) touches so many aspects of life for many people both in and out of the hospital. At our Center here at MGH, our approach is to create a true medical home where patients and their families can feel seen and heard. The greatest impact on the management of SCD comes from the establishment of a multidisciplinary care team meeting patients where they are and providing them with the resources needed to live healthy lives. With new therapies on the horizon, we have good reason to be hopeful about the future. We strive to bring the latest medical treatments and clinical trials to our Center so that our patients can have access to the most innovative approach to treating their disease.



Qualifications

QUALIFICATIONS


  • Master's of Social Work (MSW) from an accredited program required.

  • Massachusetts Licensed Independent Clinical Social Worker (LICSW) OR Licensed Clinical Social Worker (LCSW) with relevant clinical experience required.

  • Prefer experience caring for patients with chronic and/or recurring diseases and fostering long term patient relationships.

  • Ideal candidates will possess a strong interest in addressing the many factors affecting patients living with Sickle Cell Disease including implicit bias, racism, and misconceptions.
