The Department of OBGYN has an exciting opportunity for a full-time Patient Navigator to work in Miami, Fl. The Patient Navigator facilitates patients' ability to obtain services by identifying barriers to care, coordinating the development of treatment plans, and monitoring the implementation and follow-up of services ordered by caregivers.


  • Conducts patient evaluations to obtain information necessary for the development of an effective treatment plan.


  • Ensures patients have access to insurance and social service programs for which they are eligible.


  • Provides patient reminders of upcoming appointments and acts as liaison between patients and providers.


  • Identifies and resolves real or perceived barriers to care.


  • Educates and counsels patients on issues related to their health and life situations.


  • Maintains current knowledge of community health, welfare resources, and any other programs available to patients.


  • Adheres to University and unit-level policies and procedures and safeguards University assets.

MINIMUM QUALIFICATIONS



  • High School diploma or equivalent required


  • Minimum 3 years of relevant work experience required


  • Ability to process and handle confidential information with discretion.


  • Ability to communicate effectively in both oral and written form.


  • Ability to handle difficult and stressful situations with professional composure.


  • Ability to maintain effective interpersonal relationships.


  • Ability to understand and follow instructions.





DEPARTMENT SPECIFIC FUNCTIONS



  • Accompanies patients to Clinic and ER visits and visits patients admitted to the hospital as needed or requested to provide support and improve adherence to care.


  • Provides new patients with an orientation to the Clinic, including available services both on-site and in the community.


  • Provides the clinical care team with knowledge regarding the needs and concerns of the patients and assists in planning appropriate activities to address these.


  • Assists in planning promotional, educational, and other events to further the access and adherence to care.


  • Distributes educational literature and disseminates information regarding available services to targeted population.


  • Promotes services offered to the targeted group.


  • Assists in the development and administration of a patient satisfaction survey.


  • Communicates with patients effectively to identify primary concerns and provide ongoing emotional support to patients as needed.


  • Facilitates communication between the patient, physician and all members of the clinical care team.
