JOB SUMMARY:

This position leads research and programmatic efforts supporting the Health Extension for Diabetes program. Focus will be on meeting program milestones, collaborating with community and health system partners, optimizing program delivery across communities, identifying training and outreach opportunities, and supporting research and evaluation activities. Performs other duties as assigned.

JOB DUTIES:

30% - Essential - Research Administration:
Leads the research, data, and evaluation team for the Health Extension for Diabetes Program , working with faculty, extension agents, graduate and undergraduate students. Works with the HED implementation team to enhance and coordinate efforts between CU PHS, CU Extension and clinical partners. Serves as coordinator for efforts related to the ADA research program. Enabling Precision medicine: Increasing access to DSMES via an HER based clinical decision support tool (CDSP). Provides day to day operational oversight of the research project and monitor and report on the use and refinement of the CDSP and its impact on recruitment for the HED program. Serves as a key member of the HED team, coordinating program activity and leading.

20% - Essential - Research Development:
Establishes research protocols and procedures related to the ADA research program. Enabling Precision medicine: Increasing access to DSMES via an HER based clinical decision support tool (CDSP) and other HED related initiatives. Coordinates with IRB to ensure protocols for HED and associated efforts are reviewed and approved. Stakeholder Engagement, including conducting interviews and stakeholder training to improve the accessibility and awareness of a diabetes management program. Evaluates recruiting tool efficacy in ensuring access for individuals with diabetes. Uses research findings to design the program to optimize program referral, training, and communication strategies.

25% - Essential - Program Coordination and Clinical Collaboration Development:
Comprehensive knowledge of the Health Extension for Diabetes (HED) data collection methods, data reporting procedures, & program delivery protocols, advocating for the program with clinicians & funders. Collaborates closely with the HED research & program delivery team, facilitating all elements. Serves in a leadership role in the management of HED data collection efforts, including survey administration and data integrity; works with the clinical advisor for the program to identify key clinical partners & practices; improves development and implementation of the clinical decision support tool; serves as point of contact for IT & other relevant clinical teams. Coordinates w/ EPIC team & clinical leaders for diabetes care models; assists in coordination of care navigation and clinical coordination relative to HED referral and care processes; coordinates operations aimed at increasing referrals for diabetes management; studies factors enabling & inhibiting the utilization of clinical decision support tools integrated in an Electronic Health Record (EHR) system; designs tools to increase referrals from a range of sources, with a focus on providers, health systems & community partners. Ensures consistent & widespread utilization of tools that facilitate & sustain.

15% - Essential - Reporting:
Compiling and analyzing complex scientific data from research projects, ensuring accuracy and adherence to grant-specific guidelines. Drafts funder and scientific reports detailing research methodologies, results, and conclusions, effectively communicating technical information to both scientific and non-scientific audiences. Collaborates with research team to contribute to the successful submission of grant proposals and progress reports.

10% - Essential - Other Duties as Assigned supporting the work of the Clemson University of Health Research diabetes research and service team.

MINIMUM REQUIREMENTS:

Education - 1st Master's Degree
Work Exp 3+ years

PREFERRED REQUIREMENTS:

Education - 1st Doctoral / Professional Degree - Public Health or related field

Experience with diabetes education and intervention programs and familiarity with the HED program.
