Currently seeking a Social Worker, LMSW for a non-profit hospice provider in Prince George's County, MD. Organization is well established, with a great reputation for quality of care in the state of Maryland. Position offers great work life balance and the opportunity to spend one on one patient time!
Schedule: Monday - Friday 8:00AM - 4:30PM. No on-call or weekend requirements, but opportunity to pick up extra hours if desired for additional income.
Benefits and Compensation for the Social Worker, LMSW:
Highly competitive hourly rates ranging from $70,000.00 - $75,000.00 per year depending on years of experience or qualifications

Responsibilities of the Social Worker, LMSW:
Create and implement individualized plans of care for end of life patients residing in private homes and facilities across Prince George's County, MD
Provide emotional support and counseling for patients
Collaborate with interdisciplinary team (physician/provider, RN case manager, chaplain, etc) to provide the highest level quality of care.
Requirements of the Social Worker, LMSW:
LMSW, LGSW or LCSW-C Licensure is required for this role
Minimum of 1-2 years of previous clinical experience working with the geriatric patient population ideally in long term care, hospice, home health, acute care or related setting
