Job Description:

1. Thinks, communicates and behaves as a cause-driven leader and role model for other staff and members when it comes to desired staff behaviors; promotes youth development, healthy living, and social responsibility in all job-related functions.

2. Provide direct service to expectant and new moms to reduce infant mortality.
o Complete intake with either a pregnant member or a PCG of an infant under the age of one.
o Conduct follow-up with families and make appropriate referrals and linkages.
o Provide a high level of service to diverse families and ensure the Y CARES program is carried
out in a culturally competent and respectful manner.

3. Act as a community liaison throughout Franklin County.
o Secure and support referrals to and from partner agencies.
o Act as a contact for YMCA and the community health referral network in order to support individuals in the community.
o Navigate changes in referral network to ensure up to date access to resources and linkage.
o Represent the YMCA as part of community councils, coalitions, and other convening bodies to support the reduction of infant mortality and to build stronger communities.

4. Focus on improving community health of all residents.
o Plan, recruit, set up, and coordinate community health trainings.
o Attend community and YMCA outreach activities/events, in order to promote Y CARES and ECRN+ programming.
o Provide and coordinate teen health education information and referrals as needed.
o Coordinate approximately 5-10 community based outreach events each calendar year and attend additional community events held by community partners.
o Maintain a diverse and comprehensive library focused on strengthening families.
o Maintain shared calendar and spreadsheet to show program activity for each family.
o Collaborate with and contribute to physical library of resources for clients such as baby diapers, Pack n Play, and other supplies.

5. Complete required paperwork and data collection in a timely manner.
o Complete intake paperwork for each program participant within one business day.
o Ensure families are referred to appropriate resources within five (5) business days.
o Enter data into designated data system within 48 hours.
o Maintain program and client files in a confidential manner and ensure thoroughness and accuracy. Upon completion of all activities, data is entered into the designated data system within 48 business hours.
o Submit reports on all outreach activities within five (5) business days.
