Your Impact:

This position is responsible for overseeing an LCPS team that serves individuals who reside in Supportive Housing??

Communities and individual homes across Philadelphia, as well as skilled nursing facilities.???

Additionally, this position will be responsible for creating a person-centered culture of Community Support Services (CSS) service delivery, developing educational programming, and building relationships to nurture and build stronger communities within our region.? What You'll Do:
  • Interacts with individuals served, families, agencies and providers in the community to support quality services.
  • Supervises, develop, support, educate and evaluate a team of Certified Peer Specialists in day-to-day operations.
  • Provide supervision and support to Certified Peer Specialists (CPSs).
  • Oversees internal operations of LCPS program while ensuring quality programming, regulatory compliance, adherence to professional demands, and the monitoring of functional viability.
  • Ensure program meets and exceeds regulation standards and strategic goals through quality improvement processes and record reviews.
  • Perform quality assurance monitoring of services rendered and evaluated data to improve program operations and outcomes.
  • Provider direct services and support a case load as needed.
  • Educate individuals served of options, support achievement of independent living goals, and make external referrals to other agencies.
  • Offer guidance and assistance with complex situations, coordination of service needs, conflict resolutions and fair housing laws and regulations.
  • Partner with CMBS Director to provide back-up supports as needed.
  • Partner with the CMBS leadership team in support of strategic initiative implementation for CSS.
  • Partners with Community Behavioral Health (CBH) and other collaborating agencies.
  • Partners with the Business Development and Marketing and the leadership team to promote Community Support Services in the community.
  • Attends meetings and trainings as required for program operations that may include overnight travel.
  • Develops and cultivates new leaders.
  • Additional duties as assigned.
