Tennessee Community Services Agency (TNCSA) is seeking to hire a FULL TIME Independent Support Coordinator for a case load in/around the Jackson TN area. *Must be located near the Jackson, TN / Madison county area in order to conduct face to face visits when needed.
**Job can be done remotely; however, some face to face visits with clients are required.**
TNCSA provides Independent Support Coordination (ISC) services for the Department of Intellectual and Developmental Disabilities. The Independent Support Coordination Program assists individuals with intellectual and development delays to live as independently as possible within their communities through the provision of community-based services.
RESPONSIBILITIES:
Conduct meetings, writing individual support plans and submitting those plans to DIDD in order to request services, monthly monitoring of these services and completing essential paperwork.
MINIMUM QUALIFICATIONS
Bachelor's degree from an accredited college or university in social work or a related field.
Previous ISC or DIDD related experience preferred and/or certification.
Excellent written and verbal communication skills.
A valid motor vehicle operator's license is required. Must have reliable transportation.
Must authorize the release of any investigative and criminal records obtained by the Federal Bureau of Investigation and the Tennessee Bureau of Investigation to the Tennessee Community Services Agency.
