Job Description:

Working under the general direction of a collaborating physician, the advanced practice clinician (Nurse Practitioner or Physician Assistant) provides direct care to patients in a high volume clinical service. S/he develops, implements, and evaluates a program of specialized health care for a select patient population with cardiology diagnoses. S/he will be intimately involved in direct care in the form of Cardiology Consultation both urgent and non urgent for patients admitted to the Hospital. The APP will see patients throughout the hospital who need input and evaluation for various Cardiovascular issues including heart failure, arrythmias, chest pain, shortness of breath, those patients in need of preoperative clearance prior to non cardiac surgery. Responds to urgent requests for consultation and assists in expediting patients to the cath lab and other procedures in collaboration with the attending physician. The APP will work collaboratively with various members of the health care team in various specialties. S/he serves as a role model for professional medical practice in the management of patients with cardiovascular disease.

PRINCIPAL DUTIES AND RESPONSIBILITIES: Indicate key areas of responsibility, major job duties, special projects and key objectives for this position. These items should be evaluated throughout the year and included in the written annual evaluation.


• Follows and manages patients in collaboration with an attending physician in the Inpatient setting (Specifically the Cardiovascular medicine consult service ).


• Performs and records the patient history and physical exam.


• Performs Cardiology consults for patients in need in the Emergency Department and throughout the hospital.


• Utilizes knowledge & training to identify, prevent or solve complex and/or recurring patient care problems.


• Collaborates with health care professionals in other sub specialties to facilitate a comprehensive plan of care and coordinate resources.


• Writes orders which are in accord with current guidelines.


• Orders/obtains routine laboratory and related studies


• Maintains and promotes relationships with health care providers, both within the institution and outside of it.


• Serves as resource to health care professionals in area(s) of expertise.


• Participates in the development of formal and informal educational programs for health care professionals


• Monitors patient outcomes in accordance with defined standards of patient care.


• Facilitates patient movement through the health-care system and efficient use of resources through intrasystem collaboration, as well as interaction with outpatient care providers within and outside our healthcare system.


• Maintains the standards, required credentials, and educational requirements specific to the position.


• Takes responsibilities for leadership and participation on committees and task forces of the health care system.


• Maintains and updates clinical knowledge and skills based on current health care practice/research.



Qualifications

• Current Massachusetts licensure as a registered nurse in the expanded role (NP) or physician assistant


• Current Federal & Massachusetts licensures to dispense Controlled Substances


• Certification in advanced practice nursing or physician assistant


• Certification in Basic Life Support


• Certification in Advanced Cardiac Life Support


• APP experience in Cardiology preferred



WORKING CONDITIONS: Describe the conditions in which the work is performed.


Work is performed in a licensed hospital setting. The work may require the lifting of heavy objects and patients, as well as exposure to potentially infective material. Work requires contact with needles and other sharp instruments, monitoring devices, and other state-of-the-art equipment. Prolonged standing can be expected. Walking to and from the patient care units can be expected.
