RESPONSIBILITIES:
Train, develop, and supervise Health Promoters to effectively recruit participants into and maximize their participation in Fund self-management programs and individualized care coordination designed to improve health outcomes (such as the Diabetes Program, 1:1 coaching, and virtual group programs)
Monitors team goals and audits team productivity and performance to ensure the team is meeting metrics
Oversee and report on work and outcomes in participant engagement software
Manage team workflows by delegating and assigning work to staff
Participate in planning for and execution of national and regional trainings
Assist with developing and coordinating programs, policies and procedures
Coordinate with contracted UM/CM partners and healthcare practitioners in the community to optimize health outcomes and participant experience while protecting the Fund's resources (achieving the triple aim)
Maintain open communication with department leadership, informing them of employee concerns, work status, goals, resource needs, etc.
Train and develop Health Promoters to identify, recruit, and develop participant leaders to expand the reach of Fund programs and knowledge of Fund benefit
Play a key role in planning and running new or existing member events with leadership collaboration (e.g., enrollment, benefit changes, site visits)
Provide oversight for the office operations as applicable; coordinating with building managers and the local unions, including managing day-to-day operations of the office, including handling walk-ins
Oversee and assess the Health Promoter/Healthcare Navigator's skills, productivity, and performance to identify areas of strength, areas for improvement, and professional development plan and monitoring program
Work with leadership and HRBP to resolve employee performance, employee relations, attendance, and productivity issues (e.g., progressive discipline, corrective action, PIP, etc.)
Assist with and provide input to leadership on performance appraisals, promotions/demotions, etc.
Ensure the team is completing work effectively and promptly.
Evaluate the effectiveness of programs and adjust policies, procedures, and processes for continuous improvement
Plan calendar and priorities for participant engagement work in collaboration with leadership
Plan, analyzes, and evaluates programs and services, operational needs, and fiscal constraints
Supervise, lead, and delegate work and coach, mentor, and develop employees
Analyze problems, identify and develop alternative solutions, predict consequences of proposed actions, and implement recommendations/solutions
Recommend hires and promotions, direct and evaluate employment decisions for all assigned positions
Assist with developing and coordinating policies and procedures
Responsible for the oversight of continued employee training requirements, safety, and quality initiatives
Meet with, educate, and motivate participants regarding participation in Fund programs/care coordination designed to improve health outcomes
Recruit participants into Fund self-management programs; follow up with participants to ensure maximum participation; and establish regular meetings for program graduates
Meet with participants at their worksite, home, union office, Fund office, and places where they receive medical services
Identify, recruit, and develop participant leaders to expand the reach of Fund programs and train Health Promoters to do the same
Work with the union to educate, train, and develop participant leaders
Participate in meetings with the union and handle follow-up on discrete issues where appropriate
Organize and review data to identify and prioritize participants for program outreach
This is a direct-hire position in Boston. You can telecommute 2 to 3 days a week. There will be travel between 15% to 25% of the time for meetings and training. REQUIRED SKILLS:
1+ years of supervisor/manager experience (including setting and monitoring team goals and measuring performance, coaching and mentoring, providing training to staff, and conducting performance appraisals)
3+ years of direct experience as a Fund Health Promoter or as a Healthcare Patient Advocate/Care Coordinator
3+ years of direct experience in healthcare, service sector labor unions, healthcare community organizing, healthcare customer service, and/or health outreach and education
Working knowledge and experience as lead-level healthcare patient advocate/care coordinator or service sector labor union/ healthcare community organizer
MS Office, including Excel
Excellent communication skills
Ability to travel 15% to 25% of the time
PREFERRED SKILLS:
Bachelor's degree in Healthcare, Informatics, Computer Science, Economics, or a related field
Master's degree
Fluency in Haitian Creole, Spanish, or other languages
