Life's Journey Hospice is a privately owned in-patient and home hospice company located in Indianapolis, IN. We provide individualized care for life-limiting illnesses. We provide hospice care in the home, assisted living, nursing home and out inpatient unit. Our facility is designed with the hospice patient, their families, and the hospice provider in mind. Each room is equipped with large private spaces, including private patios, studio-apartment-like setups with private baths and kitchen areas. Our facility is conveniently located across the street from IU West Hospital, providing easy access for patients and families from Indianapolis and surrounding areas.
Role Description
This is a full-time role as a Hospice RN Case Manager. The Hospice TN Case Manager will be responsible for providing direct patient care, including administering medicine, managing hospice care, and providing nursing services in the home care settings. The Hospice Nurse RN Case Manager will work closely with the healthcare team to ensure the comfort and well-being of patients throughout their journey.
