Duties & Responsibilities
Completes computerized or written MDS forms and quarterly summaries within required time frame. Assures completion of the MDS by each department within the required time frame. Update care plans as needed.
Transmits completed MDS forms to appropriate state and Federal agencies at required time frame, as required by regulation.
Helps maintain communication about Med A patients between therapy staff and Hoosier Village staff; (e.g., new admissions; admit and discharge dates; RUGS; care plan schedules; and occupational therapy, physical therapy, and speech therapy patient treatment times).
Informs residents/responsible parties of Medicare covered days and notifies them of their last covered day.
Updates MDS care plan schedules.
Writes and/or assists the director of nursing in writing resident care plans for each resident. Chairs the care plan meetings.
Keeps Medicare certifications up to date.
Maintains certification to give Mantoux tests, CPR, and first aid.
Teamwork and participation: maintains an effective relationship with all departments and professional staff; and participates on facility committees as assigned.
Qualifications
Active license to practice in the State of Indiana as a LPN; RN preferred
