GENERAL SUMMARY/ OVERVIEW STATEMENT:


Reporting to the Senior Administrative Manager, the RN Nurse Coordinator is a registered nurse who is responsible for providing competent, compassionate, gender-affirming nursing care for surgical patients and their families. The RN Nurse Coordinator is responsible and accountable for the overall nursing care management of patients within the Transgender Surgical Program who are seeking gender-affirming surgery through the Division of Plastic & Reconstructive Surgery. The RN Nurse Coordinator also collaborates with other healthcare providers and attending physicians to provide optimal patient health outcomes. Exemplifies understanding and support for the transgender community.



PRINCIPAL DUTIES AND RESPONSIBILITIES:


As an integral member of the outpatient team, the RN Nurse Coordinator provides competent, compassionate, and gender-affirming nursing care based on scientific principles and the use of the nursing process. Beyond nursing care, the RN Nurse Coordinator will serve as a patient navigator, ensuring a positive experience for all patients seeking gender-affirming surgery. Represents the program to others across the hospital system and within the community.



Transgender Surgical Program


  • Orient patients to the Transgender Health Program's unique surgical model and range of services

  • Coordinate surgical processes for patients seeking gender-affirming procedures.

  • Work with financial services and Social Work to engage health insurance companies and ensure coverage for care (medical, surgical, and psychosocial as appropriate).

  • Triage and assess patient pre-operative and post-operative needs via in-person, telephone or Zoom.

  • Provide initial patient assessment and evaluation of readiness for patients seeking new/revision gender-affirming procedures.

  • Provide education and follow-up for patients exploring gender-affirming surgery.

  • Provide outreach and follow-up for high-risk patients, counseling on best post-operative care practices.

  • Provide information, education, and support to family members, referring to other clinicians or service providers as needed.

  • Serve as a resource to clinic staff and oversee the operational flow of the surgical clinic in collaboration with other nursing staff, medical assistants, and patient services coordinators.

  • Serve as a liaison between patients and other treatment team members.

  • Create and oversee patient care plans from intake and through post operative care

  • Document relevant patient information in accordance with hospital standards and utilize Epic to maintain clinical documentation.

  • Communicate the plan of care and other pertinent information to/from other health team members.

  • Primary source of triage for all Patient Gateway messages regarding Transgender surgery.

  • Review PCP surgical clearance forms.

  • See patients independently post-operatively.

  • Schedule pre-operative testing, VNA care, and pelvic floor PT.

  • Provide dilation instruction.

  • Create patient educational materials.

  • Onboard and train all new staff.

  • Oversee all modalities of communication across the multidisciplinary team

  • Coordinate and lead interdisciplinary meetings.

  • Participate in the orientation of new permanent and temporary staff members; coordinate in-service training for clinic staff.

  • Appropriately utilize nursing resources.

  • Develop and present education sessions regarding Transgender Surgery for other hospital departments, divisions, and practices.

  • Develop and present education sessions regarding Transgender Surgery for professional and academic conferences.

  • Coordinate and lead interdisciplinary patient case review meetings.

  • Provide administrative coverage for surgical coordinator as needed.

  • Utilize resources efficiently to facilitate optimal patient care.

  • Advance knowledge of transgender health and nursing best practices, successfully applying both.

  • Create/update all educational documents related to Transgender Surgery

  • Manage all dot phrases in Epic related to transgender care

  • Annual performance evaluation for TGS staff


Plastic Surgery Clinic

  • Serve as a resource to clinic staff and oversee the operational flow of the surgical clinic in collaboration with other nursing staff, medical assistants, and patient services coordinators.

  • Oversee collection of all research and PROMS data.

  • Serve on departmental and hospital committees.

  • Participate in daily operational activities necessary for a safe patient/staff environment.

  • Participate in Program and clinic efforts in operations improvement and other projects and responsibilities.

  • Interpret hospital and departmental policies and procedures to nurses and other health team members

  • Oversee all materials and supply ordering

  • Onboard and train all new staff.

  • Appropriately utilize nursing resources.

  • Annual performance evaluation for PRS Medical Assistant staff


Personnel Management



Interviews and hires assigned program support roles including medical assistants, and administrative staff.


  • Drafts and delivers annual performance evaluations for assigned staff.

  • Manages, schedules, and provides direction and feedback to assigned support staff.

  • Delegates responsibility and authority to appropriate staff and develops systems for effective follow up.

  • Provides opportunities for staff to participate in decisions related to the unit/service/department.

  • Plans, directs, and participates in regularly scheduled meetings to share information and problem solve with unit personnel.

  • Implements all levels of corrective action according to hospital policy as needed.

  • Resolves employee relations situations, in conjunction with Human Resources as necessary.

  • Collaborates with service and other educational resources to facilitate learning experiences for staff.

Operations Management



  • Maintains a safe environment for patient/staff while executing daily operational activities necessary for effective dermatology practice functioning.

  • Implement unit QA projects to improve patient outcomes.

  • Helps define standards of excellence for patient care; participates in improvement of patient care services in a cost-effective way.

  • Participates in planning current and long-term projects to improve the operation of the Dermatology Surgery Unit

  • Oversee all prior authorization required for patient medications and oversee the PA process for procedures.






Qualifications

QUALIFICATIONS:





  • Must be a graduate of an accredited nursing program.

  • Must have a current Massachusetts license as a Registered Nurse.

  • Bachelor's Degree required.

  • At least two (2) years of experience in Ambulatory nursing and (1) year of experience in coordinating surgical care

  • Prior experience with the transgender community is preferred




SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:





  • Established and demonstrated skills and knowledge within nursing practice and with an understanding of surgical services and care.

  • Understanding of the transgender community and clinical needs especially as related to gender-affirming surgery.

  • Possesses excellent assessment, critical thinking, judgment, and interpersonal skills.

  • Deals with and cares for complex physical, clinical, and psychosocial needs of patients and families

  • Prioritizes and manages time efficiently and utilizes effective organizational skills, ensuring appropriate follow-up with staff, patients, families, agencies, and others.

  • Able to utilize technologies such as Epic EMR, Zoom, and other resources to make sure patient care is well documented

  • Works effectively in an independent manner and within the scope of professional practice guidelines

  • Works effectively within a multidisciplinary team and practice setting.

  • Able to represent the Program in various settings within the hospital and community and to articulate the services, goals, and resources.

  • Demonstrates effective teaching skills necessary for patient care and staff mentoring




WORKING CONDITIONS:



• Office setting in an outpatient medical clinic with potential exposure to sharps, bodily fluids, and other clinical hazards.


• Some remote work may be incorporated in compliance with infection control/COVID-19 efforts



SUPERVISORY RESPONSIBILITY:



• Supervise 1 to 6 FTEs, comprised of Medical Assistants and Patient Service Coordinators
