Successful candidate must have solid experience & advanced knowledge of:
High School diploma or GED.
Advanced computer and data entry skills
Valid driver's license
Basic Life Support Certification (can be obtained after hire at SCHC)
Mandatory Reporter Certification for Child & Dependent Adult (can be obtained after hire at SCHC)
Complete Professional Skills Training for Community Health Workers within a year of hire.
Complete health coach training (Fusion) and pass motivation proficiency within two years of hire.
Successful candidate must be able to perform primary functions of position:
Assist patients in navigating health system at the Health Center, County, or State level.
Access and communicate barriers in navigation to superiors.
Provide basic education on need for screening tests, self- management of chronic diseases/wellness.
Contact patients under supervision of clinicians to assess the need for additional services, compliance with treatment programs/plans.
Travel to patients' homes to assess barriers and help coordinate the reduction of those barriers.
Assist quality staff with data projects and assist marketing staff with community events.
