Job Title: Hospice RN Case Manager



Duties & Responsibilities: Hospice RN Case Manager sought for 6 or 12 week assignment, with the potential to extend. Schedule would be 5x8 shifts Monday - Friday.



Minimum Requirements:

• CT RN license


• BLS


• 1 year hospice in home settings experience within the past 3 years
