Social Worker/ LICSW/ LCSW/ 40 hours/ Full Time/
Post- Abdominal
TRANSPLANT/ MGH Boston-




  • We are seeking a social worker to join our Abdominal Transplant Team in the MGH Transplant Center.


  • $3,000 sign-on bonus for LCSW, based upon eligibility (non-MGB)


  • $7,000 sign-on bonus for LICSW, based upon eligibility (non-MGB)


  • Excellent clinical, collaborative and consultation skills required to offer social work services to patients and families as part of a multidisciplinary team. Responsibilities include assessment, crisis intervention and counseling.


  • In addition, participation in Transplant Center activities, selection committee meetings, group facilitation, and program development is necessary.


  • Hybrid Schedule, with onsite and virtual/remote schedule possible.


LICSW preferred. LCSW with relevant experience will be considered.



About this Post Transplant social worker role:


  • Provides psychoeducation, clinical services, and relevant community resources that address personal and environmental issues to help optimize patient/family coping and emotional/social/physical well-being after organ transplantation.

  • Uses clinical knowledge of psychosocial dynamics and trauma-informed care to conduct psychosocial evaluations when indicated, identify barriers to optimal engagement in post-transplant care, and assist patients/families in developing and implementing treatment goals.

  • Provides consultation and education to interdisciplinary staff regarding the social and emotional aspects of patient care, adherence, and treatment. Advises and addresses ethical and risk management concerns.

  • Engage in program development such as creating support groups and other initiatives to enhance support and education for patients/families.

  • Participates in clinical supervision as appropriate to level of professional development. Encourages an environment of idea sharing and feedback. Seeks opportunities and implements goals for own professional development.

Principal Responsibilities


  • Delivers professional and thorough social work services, including psychosocial assessment and intervention planning.

    • Identifies patients who require social work assessment and intervention through high risk screening, interdisciplinary team meetings and individual referrals.

    • Conducts assessments that address bio-psycho-social issues for age, population and health specific needs which results in individualized plans of care.

    • Provides support and counseling to patients and families.

    • Provides information and assistance for identified financial or social needs.



  • Coordinates complex transition plans with patients, families, health care team and community providers.

    • Utilizes the electronic medical record to monitor, document and communicate patient progress toward goals and progression of the social work plan.

    • Collaborates and communicates with interdisciplinary team anticipating needs to move the plan of care forward.

    • Provides support and information to patient and families regarding transition plan.

    • Maintains knowledge of government and private payer networks and services to assure appropriate transitions.

    • Collaborates with community and health care resources based on need to coordinate care for the patient.



  • Advocates for patients and families by supporting patient rights and accessing protective services.

    • Demonstrates awareness of patient rights and ethical decision making; provides advocacy to support patient and family.

    • Assures appropriate reporting of vulnerability or suspected abuse as mandated by law.



  • May participate in care system process that prevent readmissions.

    • Plans and participates in transition conferences with patients and families.

    • Utilizes tools and technology to identify and intervene with patients who are at risk for readmission.

    • Ensures that a complete clinical handoff occurs for at risk patient, which may include referrals.



  • May collaborate with the health care team to promote appropriate length of stay.

    • Utilizes tools and technology to support appropriate length of stay management.

    • Facilitates timely referrals and transfers of information.

    • Other duties as assigned.









Qualifications

QUALIFICATIONS

  • Master's of Social Work (MSW) from an accredited school of social work.

  • Require current Massachusetts Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Social Worker (LCSW), or must obtain MA Licensed Clinical Social Worker (LCSW) within 4 months of hire date.

  • Strong assessment and treatment skills.

  • Excellent organizational skills.

  • Demonstrates ability to communicate effectively orally and in writing.

  • Strong interpersonal skills enabling effective collaboration.

  • Demonstrates ability to understand the role of social worker in a health care setting and to work independently in a complex, fast-paced environment.
