Responsibilities:

• Conduct comprehensive evaluations of residents to assess their physical condition and functional abilities.


• Develop individualized treatment plans tailored to the unique needs and goals of each resident.


• Administer therapeutic exercises, manual techniques, and modalities to improve mobility, strength, and range of motion.


• Collaborate with interdisciplinary team members to coordinate care and achieve optimal outcomes for residents.


• Educate residents and their families on therapeutic techniques, home exercises, and injury prevention strategies.


• Maintain accurate and timely documentation of treatment sessions, progress notes, and discharge summaries.

• Current licensure as a Physical Therapist in the state of California.


• Previous experience in a skilled nursing or rehabilitation setting preferred.


• Strong clinical skills and proficiency in conducting assessments, developing treatment plans, and implementing evidence-based interventions.


• Excellent communication and interpersonal skills, with the ability to effectively collaborate with team members and interact with residents and families.
