About the Position:
Review admission health histories and develop nutrition, childbirth, and general health plans with each resident
Ensure timely communication and planning for all health and medical related information to Residential staff for new admissions
Obtain admission health histories and develop nutrition, childbirth, and general health plans with clients
Assess acute medical needs of clients in the program and document medical/nursing assessment and intervention with residents in nursing charts
Coordinate clients preventative and acute health care needs with obstetric, pediatric, and primary care providers and assist in overseeing their medical care
Obtain primary, dental, eye, and other specialty care for clients as needed
Plan and implement a comprehensive prevention-focused health curriculum and educational opportunities for clients including on topics such as prenatal care, childbirth education, preventative health care, wellness, STI's, contraception and family planning, infant care and nutrition, developmental and cognitive growth of children, and supporting parents to gain skills in advocating for self and child in health care
Maintain strict accuracy of the Medication Administrative Record (MAR), including documentation of daily medication dispensing, as well as timely recording of all medication changes and additions
Administer client medications
Provide transportation and accompany clients to medical and dental appointments, assist clients with information integration, and incorporate new medical information into existing client treatment planning
Complete all required paperwork within established timelines, including, but not limited to discharge summaries and daily client and parenting logs as appropriate
Coordinate medical discharge planning needs for clients including appropriate community referrals and attendance at discharge planning meetings
Provide individual education and support to parents in the health care of their infants/children
Assess daily acute medical needs of parents and children in the program and document medical/nursing assessment and interventions in EHR
Provide health advocacy for clients and support them in the development of self-advocacy skills
Ensure responsible fiscal management through such practices as careful management of existing stock
Attend weekly clinical and treatment team meetings
Other duties as assigned
What We Look For:
Graduation from an accredited college/university with a nursing degree and current licensure as a registered nurse in the state of Vermont
Bachelor's Degree in a related field required
Ability to perform the essential functions of the position either with or without reasonable accommodation
Ability to maintain a valid VT Driver's License and access to reliable transportation. Occasional statewide travel may be required
Upon conditional offer of employment, must be able to successfully pass a background check
