Job Description:
Location: Portland, OR
Description: Our client is currently seeking a Nurse Practitioner - OREGON
Opportunity: Nurse Practitioner
Location: Multiple counties to include, Benton, Linn, Marion, Polk, Yamhill, Clackamas, Washington, Multnomah
Contract assignment: 4-7 months
Responsibilities:
- Effectively manage acute and chronic medical and behavioral conditions.
- Ensure accurate and complete documentation, including ICD-10 conditions.
- Collaborate with the interdisciplinary care team to provide comprehensive patient care.
- Conduct advanced illness and advanced care planning conversations.
- Offer counseling and education to patients and caregivers.
Qualifications & Requirements:
- Active and unrestricted license in the state of Oregon
- For Physician Assistants (PA): Graduate of an accredited Physician Assistant degree program.
- For Nurse Practitioners (NP): Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following: Family Nurse Practitioner, Adult Nurse Practitioner, or Gerontology Nurse Practitioner
