Job Title: Licensed Master Social Worker
Responsibilities:
Screen all new admitted patients on Mother/Baby using a high-risk assessment tool.
Assess patients' needs for services and thoroughly document findings in the medical record.
Conduct psychosocial assessments on patients at risk and develop comprehensive treatment plans.
Provide a range of services, including supportive, educational, individual, family, and group counseling.
Collaborate with interdisciplinary teams to plan for the care of infants and make referrals for concrete services as needed.
Offer crisis intervention and individual/group services as indicated by patient needs.
Requirements:
Minimum of 1 year of experience as a Licensed Master Social Worker (LMSW).
Valid Licensed Master of Social Work (LMSW) license issued by NYSED, with Primary Source Verification.
Experience in the Mother/Baby department, with a strong understanding of patient care and discharge planning.
Master's degree in social work or related field.
