HCAM offers diverse connection and coordination services for individuals and families across Maryland through various programs. HCAM’s Care Coordination Program (CCP) provides short-term care coordination services to Baltimore City residents who are eligible for Medicaid benefits and need help accessing services. The Care Coordination Program strives to improve birth outcomes and protect our city’s most vulnerable populations in Baltimore City though outreach strategies and providing access to health services and community resources by:

 


  • Assisting Medicaid/HealthChoice eligible beneficiaries in accessing, and maximizing their Medicaid benefits

  • Educating clients about health benefits

  • Linking residents to primary care and specialty services


The RN Care Coordinator will serve as a part of CCP’s Administrative Care Coordination Unit (ACCU) and will provide short term care coordination services to individuals and families. 



DUTIES and RESPONSIBILITIES:

 

  1. Develop and facilitate services and partner with community resources to address the needs of assigned populations

  2. Provide support and expertise through assessment, planning, implementation and overall evaluation of the individual client needs.

  3. Perform as a Team Lead responsible hosting team meetings with outreach staff members to discuss complex cases, best practices, and sharing of resources.

  4. Receive and outreach referred clients that have identified medically or psychosocially complex needs. Provide education of Medicaid benefits and services, re-connect recipients to medical home, Managed Care Organization, and linkage to covered benefits.

  5. Assess prenatal/postpartum client risk and eligibility for referral and enrollment into B’More for Healthy Babies Centralized Intake services including WIC, home visiting programs, parenting support groups and Safe Sleep.

  6. Provide appropriate resource linkage to MCO incentive programs and case management, Medical Assistance Transportation.

  7. Consult with other community agencies to identify potential resources for resolving client’s health, psychosocial or financial problems.

  8. Deploy various methods for locating referred clients that are difficult to locate, establishing liaisons with DSS, PCP’s, neighbors, churches, and other community groups.

  9. Function as a liaison to external agencies and shares information with others, which may impact care and/or services of clients and referral services.

  10. Participates in staff, community, professional and inter-agency meetings, and conferences.

  11. Documents client assessments, activity, service plans and resolutions; prepares narrative and statistical reports.

  12. Perform other appropriate duties as assigned.

 

Minimum Qualifications


  • A Bachelor’s degree in Nursing from an accredited college or university.

  • Current licensure as a Registered Nurse in the State of Maryland.

  • A minimum of 2 years related experience

  • Relevant clinical experience providing services to pregnant women and/or infants, preferably in a public health/community setting.


KNOWLEDGE, SKILLS and ABILITIES:
 

  • Strong computer literacy skills, specifically with MS Word, Excel, Outlook, and SharePoint (365).


  • Organizational skills with the ability to meet a demanding workload.

  • Excellent oral and written interpersonal and communication skills required.

  • Problem solving skills.

  • Proven ability to work efficiently and meet deadlines.

The employee must regularly lift and/or move up to 10 pounds and occasionally lift and/or move up to 15 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

While performing the duties of this job, the employee is exposed to both indoor and outdoor conditions. The employee will be subject to noise.
