Job Description:

DHHS Public Health is seeking a Community Health Educator to provide Heart Disease and Diabetes prevention, outreach, education, and intervention to meet the needs of Nebraska communities.


These chronic disease prevention programs offer lifestyle interventions to help Nebraskans avoid or manage a Diabetes or Cardiovascular disease diagnosis and distribute and present information on chronic disease to communities and partners to support healthy living across Nebraska.


Job Duties:


*Support coordination of the Heart Disease and Stroke Prevention and Management Nutrition and Physical Activity focused initiatives, and Diabetes Prevention and Management Programs throughout the state.


* Be a community partner liaison between our federal funding sources and programs in the community.


* Support outreach to new or repeat partners who have an interest in implementing our programs, and facilitate and monitor partner touchpoints.


* Develop and manage grant and contract agreements; complete budget monitoring and planning; develop program policies, procedures, and evaluation.


* Provide ongoing program quality assurance and monitoring and ensure compliance with state and federal regulations.


* Research and write required plans, grants, and proposals; and maintain internal databases and resources.



Requirements/ Qualifications:




Minimum Qualifications:Bachelor's degree or equivalent coursework/training in community health education, biological science, adult/continuing education or related field AND experience in public/community health education activities with responsibility for: developing, evaluating, and leading health education program activities; assessing health needs; writing and designing educational and informational material, training aids, and presentations; preparing grant proposals; or planning and leading group sessions. Related experience may substitute for a Bachelor's degree on a year-for-year basis



Preferred Qualifications: Master's degree in public or community health, healthcare, or related field. One year of experience working in public or community health, healthcare, or related field. Experience or knowledge of health care system interventions to improve the delivery and quality of care in clinical settings that lead to early detection, management, and prevention of heart disease and stroke and Diabetes prevention and control; and connecting community programs with health systems to improve heart disease and stroke prevention, care, and management.



Knowledge, Skills, and Abilities:




Knowledge of the principles and practices of public health planning and implementation; of grant writing, reporting, and grant administration; and of developing partnerships and collaborations.



Ability to maintain accurate and thorough records, design educational and informational materials, training aids, and presentations; plan and lead group sessions; communicate effectively;



Ability to prioritize and manage time; problem solve and make decisions; demonstrate professionalism; balance needs, requirements, and workload for multiple projects;



Ability to interpret and apply federal grant requirements and prepare grant proposals as well as agency rules, policies, and procedures;
