Description: Judge Healthcare is actively seeking 3 dedicated and compassionate Licensed Social Workers for a facility based throughout Massachusetts. As a Licensed Social Worker, you will play a crucial role in planning, organizing, implementing, evaluating, and directing the facility's Social Services Department. This job will have the following responsibilities:

• Assist in planning, developing, organizing, implementing, and evaluating the facility's social service programs.


• Participate in discharge planning, resident assessments, and the development of social care plans.


• Interview residents and their families to obtain social history.


• Refer residents and families to appropriate community agencies when needed.


• Provide consultation to staff and community agencies to meet residents' social and emotional needs.


• Interpret residents' social, psychological, and emotional needs to the care team.

Qualifications & Requirements:

• Bachelor's degree in Social Work


• Minimum 1 year of supervised experience in healthcare


• Massachusetts Social Work Licensure
