Job Description:

Nystrom & Associates, Ltd. is a team of mental health professionals that specialize in psychiatric evaluations, clinical social work, and family therapy. We are committed to helping patients in our community who are experiencing personal, emotional, marriage, family or psychological problems. Founded in 1991, we offer an intensive outpatient chemical dependency program, psychological testing, and behavioral health and medication management services.

As a Patient Care Navigator at Nystrom & Associates, you will assist telehealth providers with rooming patients and taking vital signs, meaningful use tasks related to electronic health records, assists with administrative staff tasks and duties.

Fulltime schedule: Monday - Friday 7:15 AM - 3:45 PM

Pay rate: $16.50 per hour

Job Duties:

Greet patients in a professional, timely, and amiable manner.
Obtain vital signs such as: height, weight, temperature, blood pressure, pulse.
Obtain and process urine and saliva drug screens.
Maintain accurate and organized patient records in accordance with Nystrom policy and procedure.
Escort patient to meet with provider and facilitate warm introduction with Nystrom provider.
Aid in completing patient paperwork and medication verifications.
Aid in completing voicemail transcription, lab transcription, and other duties assigned.
In office and virtual support of surrounding clinics.
Assist provider and Psychiatry Triage Team with crisis interventions.
Scheduling of appointments in Allscripts PM.
Collegial and team-work attitude with all staff and patients.
Keep office space orderly.
Follow clinic policies and procedure.
Handle client complaints and concerns or redirect to supervisor.
Inform supervisor of any problems or items that need to be ordered.
All other duties assigned by management not included in this position description.


Qualifications:

Prior experience working in health care facilities.
Ability to multi-task
Ability to manage time well.
Positive attitude and ability to be flexible.
Must be proficient in use of computers, including Microsoft Teams and Nystrom EEHR.
Professionalism in verbal and written communication
