The APRN ensures proper illness and injury care and disease prevention, diagnosis, treatment, and recovery. The APRN may prescribe medications, order diagnostic tests, and advise patients about continuing carein-person or virtually, according to BPEI protocols.


CORE JOB FUNCTIONS




  • Performs and documents complete physical examinations and comprehensive health histories.


  • Orders and interprets diagnostic and therapeutic tests relative to patient's specific needs.


  • Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.


  • Implements interventions to support the patient to regain or maintain physiologic stability.


  • Assists with the provision of care in accordance with facility, state, and federal regulations.


  • Monitors the effectiveness of interventions.


  • Facilitates the patient's transition within and between health care settings, e.g., admitting, transferring, and discharging patients.


  • Collaborates with multidisciplinary team members by making appropriate referrals.


  • Facilitates staff, patient, and family decision-making by providing educational tools.


  • Adheres to University and unit-level policies and procedures and safeguards University assets.

Ophthalmology Advanced Practice Registered Nurse (APRN)



Bascom Palmer Eye Institute (BPEI)


CORE JOB SUMMARY


The Advanced Practice Registered Nurse (APRN) delivers medical care to a wide variety of patients. The APRN also examines and treats patients in collaboration with other healthcare professionals. The APRN ensures proper illness and injury care and disease prevention, diagnosis, treatment, and recovery. The APRN may prescribe medications, order diagnostic tests, and advise patients about continuing care.


The principles of APRN practice include collaborative patient care, a holistic approach to intervention, clinical communication, post-procedure follow-up, medication management, clinical scheduling efficiency, continuity of care, promotion of virtual care, and outreach follow-up. This role significantly enhances service efficiency, patient satisfaction, and overall clinical outcomes.


The APRN participates in BPEI quality of care and clinical outcomes activities and complies with accrediting and regulatory standards. S/He consults with UHealth departments to facilitate patient safety across care levels. S/He participates in all professional growth and development activities, including mandatory in-services and computer-based learning modules. The APRN participates in research projects as determined by the specialty.



Department-Specific Functions



Care Collaboration




  • Collaborate in the plan of care for patients and physicians in high-acuity clinics, including ordering and reviewing diagnostic tests and images and providing patient education.


  • Manage/co-manage non-acute, chronic/long-term patients in collaboration with specialty physicians.


  • See selected new patients as agreed upon with collaborating physician.


  • Maintain and track surveillance testing for specialty patients.


  • Assist patient care coordination with other eye centers, affiliated hospitals, UHealth Tower consultative services, and Rapid Access Clinics programs.


  • Follow up with patient post-procedure to ensure patient satisfaction and address any concerns.


  • Facilitate referrals to subspecialties and other referral requests.


  • Route international patients and second opinion requests per specialty protocol.


  • Promote continuity of care across levels of care settings and during the absence of specialty physicians, such as during vacation periods. This involves caring for urgent patients and maintaining treatment consistency.





Medication Management




  • Provide medication reconciliation and refills for specialty patients as indicated by physicians.


  • Assist with peer-to-peer evaluations for patients needing approval for medications.


  • Manage medication refill requests and related patient education, thereby enhancing patient convenience and adherence to treatment plans.





Clinic Efficiency




  • Review clinic schedules in advance to appropriately divert patients from MD services due to insurance constraints, prioritizing new patient slots for high acuity and surgical patients.


  • Review patients' history and current diagnostic testing to develop patients' plans of care for upcoming visits.


  • Order appropriate baseline and pre-op testing based on the patient care needs.


  • Maintain and track surveillance testing for patients per specialty.


  • Provide logistical assistance to support the patient's needs while working closely with the administrative assistant.


  • Assist in prior authorizations and managing non-urgent medical paperwork, thus easing the administrative burden on medical staff.





Clinical Facilitation




  • Appropriately triage patient calls for both clinic and preoperative patients. Consult with fellow and attending physicians.


  • Manage clinically related phone queries screened by administrative assistants to provide timely responses and convert these interactions into telemedicine sessions when appropriate.


  • Facilitate communication between physicians and referring doctors.


  • Based on the specialty, assist with laser procedures in clinical settings.


  • Monitor patients' test results using remote monitoring devices (RMDs) and record the results in the patient's electronic medical record.


  • Communicate with patients about their RMD measurements and coordinate virtual and in-person follow-up care when needed.


  • Utilize the virtual hub to implement virtual first or virtual follow-up appointments.


  • Assist in the follow-up of patients identified at community screening events who require further medical or surgical care to bridge the gap between initial screening and comprehensive evaluation.


  • Facilitate patient referral back to the community.





Patient Education and Support




  • Participate in patient and family counseling/education.


  • Provides multi-discipline/program resources, education, psychosocial care, and logistical assistance to support patient and family needs.


  • Keep abreast of the most recent and relevant specialty information.


  • Provide patients and families with information and teaching for the use of RMDs.


  • Develop appropriate policies and programs (e.g., educational sessions for clinical coordinators, photographers, technicians, etc.) after appropriate training.


  • Develop specialty-specific patient education programs.


  • Collaborate with educational trainers on the use of telemedicine-related equipment for both staff and patients.
